阳和汤加味治疗寻常型银屑病临床研究

摘　　要：目的:观察阳和汤加味治疗寻常型银屑病的临床疗效。方法:选取河南科技大学第一附属医院皮肤科2015年12月―2018年12月收治的120例寻常型银屑病患者,分为观察组与对照组各60例。对照组采用窄谱中波紫外线(NB-UVB)治疗,观察组在对照组的基础上使用阳和汤加味治疗,疗程均为4周。比较2组的临床疗效,治疗前、治疗2周后及治疗4周后2组患者银屑病皮损面积严重程度(PASI)评分及中医证候评分。结果:治疗4周后,观察组与对照组患者临床总有效率分别为95.00%、81.67%,观察组显著高于对照组(P<0.05)。治疗2周后,2组患者PASI评分较治疗前显著降低(P<0.05),2组间比较,差异无统计学意义(P>0.05);治疗4周后观察组患者PASI评分较治疗2周后显著降低(P<0.05),且观察组低于对照组(P<0.05)。治疗2周后2组患者中医证候积分较治疗前显著降低(P<0.05),且观察组低于对照组(P<0.05);治疗4周后观察组中医证候积分较治疗2周后显著降低(P<0.05),且观察组低于对照组(P<0.05)。结论:阳和汤加味治疗寻常型银屑病可有效减轻患者皮损严重程度和皮损面积,改善患者临床症状。Objective:To observe the clinical effect of modified Yanghe tang for psoriasis vulgaris.Methods:A total of120 cases of patients with psoriasis vulgaris were admitted and treated in department of dermatology in the First Affiliated Hospital of Henan University of Science and Technology from December 2015 to December 2018,and were divided into the observation group and the control group,60 cases in each group.The control group was given narrow-band ultraviolet B(NB-UVB),and the observation group was additionally given modified Yanghe tang based on the treatment of the control group.Both groups were treated for four weeks.The clinical effect as well as the scores of Psoriasis Area and Severity Index(PASI)and the Chinese medicine syndrome scores before treatment and after two-week and four-week treatment was compared between the two groups.Results:After four-week treatment,the total clinical effective rate was 95.00% in the observation group,significantly higher than that of 81.67% in the control group(P<0.05).After two-week treatment,the scores of PASI in the two groups were significantly decreased when compared with those before treatment(P<0.05),there being no significance in the difference among the groups(P>0.05);after four-week treatment,the score of PASI in the observation group was significantly decreased when compared with that after two-week treatment(P<0.05),and lower than that in the control group(P<0.05).After two-week treatment,the Chinese medicine syndrome scores in the two groups were significantly decreased when compared with those before treatment(P<0.05),and the score in the observation group was lower than that in the control group(P<0.05);after four-week treatment,the Chinese medicine syndrome score in the observation group was significantly decreased when compared with that after two-week treatment(P<0.05),and lower than that in the control group(P<0.05).Conclusion:Modified Yanghe tang in the treatment of psoriasis vulgaris can effectively reduce the severity and area of skin lesions of patients
