摘  要:目的分析瘦素(LP)基因-2548 G/A位点多态性及其血清LP水平与DKD患者的相关性。方法选取2016年1月至2017年10月于胜利油田糖尿病医院内分泌科门诊及住院治疗的200例单纯T2DM患者(T2DM组)及200例DKD患者(DKD组),利用聚合酶链反应限制性片段长度多态性技术评估各组LP-2548G/A基因型分布,并检测血糖、脂质谱、肾功能、FIns和LP水平。结果AA基因型患者LP和UAER水平高于其他基因型患者(P<0.05)。DKD组LP与血肌酐、BUN、UAER及FIns呈正相关(P<0.05)。GA、AA基因型患者患DKD风险较其他患者增加2~3倍(P=0.032、0.009)。Logistic多因素回归分析显示,AA基因型是DKD发生的危险因素(OR=1.584,95%CI 1.30~8.32,P=0.037)。结论LP-2548G/A多态性与DKD发病有关,多态性可能影响LP水平。ObjectiveTo investigate the association between-2548 G/A polymorphism in leptin gene and diabetic kidney disease(DKD)in patients with type 2 diabetes mellitus(T2 DM).MethodsA total of 200 T2 DM patients(T2 DM group)and 200 T2 DM patients with DKD(DKD group)who were treated in the Endocrinology Department of Shengli Oilfield Diabetes Hospital from January 2016 to October2017 were selected in this study.The leptin-2548 G/A genotype was evaluated by polymerase chain reaction restriction fragment length polymorphism in each group.Blood glucose,lipid profile,renal function,insulin and leptin levels were detected in all the subjects.ResultsSerum leptin and UAER levels were higher in patients with AA genotype than in patients with other genotypes(P<0.05).In DKD group,leptin was positively correlated with serum creatinine,urea,UAER,and insulin(P<0.05).GA and AA genotypes could increase about 2 and 3 fold of renal disease risk,respectively(P=0.032,0.009).Logistic multivariate regression analysis showed that AA genotype was a risk factor for DKD(OR=1.584,95%CI 1.30~8.32,P=0.037).ConclusionThe leptin-2548 G/A polymorphism was associated with DKD,and this polymorphism may affect leptin level.
