直出式绿激光前列腺剜除术与电切术治疗良性前列腺增生的对比研究

摘　　要：目的探讨经尿道直出式绿激光前列腺剜除术的临床效果及术后并发症的特点。方法选取2016年1月至2019年1月收治的110例患者为研究对象,分为两组,剜除组55例,行经尿道直出式绿激光前列腺剜除术;电切组55例,行经尿道前列腺电切术。将两组手术时间、术中失血量、导尿管留置时间、术后住院日、腺体切除重量、术后住院期间并发症(术后出血、尿路感染、术后尿潴留)情况进行对比观察。结果剜除组在术中失血量、导尿管留置时间、术后住院日、腺体切除重量等方面优于电切组,差异有统计学意义(均P<0.05),两组间手术时间差异无统计学意义(P>0.05);术后住院期间并发症总发生率为剜除组3.64%,电切组23.64%,差异有统计学意义(均P<0.05)。结论尿道直出式绿激光前列腺剜除术切除增生腺体较彻底,术中出血少,对腺体周围正常组织损伤小,术后恢复快,更加安全。Objective To investigate clinical difference between transurethral direct green laser enucleation of prostate and transurethral resection of prostate in treatment of benign prostatic hyperplasia.Methods From January 2016 to January 2019,110 patients with benign prostatic hyperplasia in our hospital were selected in the program and divided into transurethral direct green laser enucleation of prostate group(n=55)and transurethral resection of prostate(TURP)group(n=55)randomly.The differences between two groups in following aspects,such as operation time,intraoperative blood loss,catheter indwelling time,postoperative hospital stay,amount of prostate tissue resected and surgical complications were observed.Results The enucleation group was significantly superior to the TURP group in terms of intraoperative blood loss,catheter indwelling time,postoperative hospital stay and the weight of resected prostate(mean P<0.05).The difference between two groups in the operative time was not statistically significant(P>0.05).The incidence rate of postoperative complications was 3.64%(2/55)in the enucleation group and 23.64%(13/55)in the TURP group,and there was obvious difference between two groups(P<0.05).Conclusions The total effect of transurethral direct green laser enucleation of prostate in the therapy of benign prostatic hyperplasia was better than that of TURP group,due to the less damage on normal tissues near the prostate,faster postoperative recovery and higher safety.
