绿激光治疗前列腺增生术后下尿路梗阻症状复发的手术技巧及疗效探讨

摘　　要：目的探讨经尿道绿激光治疗前列腺增生术后下尿路梗阻症状复发的临床疗效。方法回顾性分析我院2018年1月至2018年12月采用绿激光治疗的21例经尿道前列腺电切术后下尿路梗阻症状复发的病患临床资料,比较手术前后尿流动力学参数、IPSS评分及QOL评分。结果21例患者均顺利完成手术,手术时间5~30 min,平均18.5 min,术中未输血。术后随访3~12个月,所有患者术后均排尿通畅。16例有尿流动力学检查,术前最大尿流率(Qmax)为(3.44±1.81)ml/s,术后1月Qmax为(19.19±3.17)ml/s,手术前后尿流动力学指标差异有统计学意义(P<0.05)。术前IPSS评分(31.65±2.53)分;QOL评分(5.15±0.50)分;术后3月复测IPSS评分为(5.69±1.40)分;QOL评分(1.93±0.68)分,手术前后指标差异有统计学意义(P<0.05)。结论绿激光治疗良性前列腺增生术后下尿路梗阻症状复发疗效确切,手术时间短,出血量少,术后恢复快,是一种安全有效的治疗方法。Objective To explore the clinical effectiveness of green-light laser in treating recurrent symptoms of lower urinary tract obstruction after transurethral resection of prostate.Methods The clinical data of 21cases with recurrent symptoms of lower urinary tract obstruction who were performed operation at First Affiliated Hospital of Anhui Medical University from Januray to December in 2018 were retrospectively reviewed.Perioperative and postoperative characteristics including International Prostate Symptom Score(IPSS),maximal flow rate(Qmax)and quality of life(QOL)were compared.Results Sucessful operations were performed on all 21 cases,and the average opertation time was 18.5 min(5-30min)without blood transfusion.All patiens urinated smoothly during follow-up period of 3 to 12 months after operation.The data of 16 cases with urodynamic examination before and after operation was analyzed.Perioperative and postoperative Q max were(3.44±1.81)ml/s and(19.19±3.17)ml/s respectively,which was statistically significant.Perioperative and postoperative IPSS score were(31.65±2.53)and(5.69±1.40)respectively(P<0.05).Perioperative and postoperative QOL score were(5.15±0.50)and(1.93±0.68)respectively(P<0.05).Conclusions Green-light laser is verified as a safe and effective way in treating recurrent symptoms of lower urinary tract obstruction after transurethral resection of prostate with short operation time,little hemorrhage and fast recovery.
