摘  要:目的核糖核酸[如信使核糖核酸(mRNA)和微核糖核酸(miRNA)等]可在尿液标本中检测到,但尿液是复杂的混合物,包含尿沉渣和尿外泌体等组分.目前,仍不清楚哪种组分之中的核酸能够更直接准确地反映肾纤维化情况.方法我们对健康志愿者和肾纤维化患者,检测和分析了4个不同的肾纤维化相关核糖核酸(RNA),以及miRNA内参基因U6和mRNA内参基因β-肌动蛋白(β-actin)在尿沉渣和尿外泌体中的相对含量.结果定量逆转录聚合酶链式反应(qRT-PCR)检测结果显示,在健康志愿者中,尿外泌体和尿沉渣中RNA水平相似.但方差齐性检验(F检验)结果显示,部分尿外泌体源RNA比尿沉渣中相应RNA含量更稳定,结果变异更小,且在健康志愿者和肾纤维化患者中这些相对稳定的RNA不同,前者为miR-29c、miR-21和E-钙黏附蛋白(E-cadherin),后者为miR-29c和E-cadherin.在肾纤维化患者,miR-29c水平在外泌体中含量高于尿沉渣中含量,差异有统计学意义(P<0.05).结论尿外泌体可能是一种比尿沉渣更好的肾纤维化生物标志物检测对象.肾纤维化患者尿外泌体源miR-29c可能是一项较好的检测指标.Objective Ribonucleic acid(such as mRNA and miRNA) can be detected in urine samples. However, urine is a complex mixture including urinary sediments and urinary exosomes. It is still unclear the nucleic acids in which components can more directly and accurately reflect renal fibrosis. Methods We assessed and analyzed the relative contents of four different renal fibrosis related RNA and the common housekeeping gene of miRNA(U6) and mRNA(β-actin) in urinary sediments and exosomes. Results The results of qRT-PCR showed that the levels of RNA in urinary sediments and exosomes were similar in healthy volunteers. But F test showed that some RNAs in the urinary exosomes were more stable than those in urinary sediments. The relatively stable RNAs varied in healthy volunteers and patients with renal fibrosis, the former included miR-29 c, miR-21 and E-cadherin, and the latter were miR-29 c and E-cadherin. In patients with renal fibrosis, the relative level of miR-29 c in the exosomes was significantly higher than that in the urinary sediment(P<0.05). Conclusion Urinary exosomes may be a better biomarker for renal fibrosis detection than urinary sediments. Urinary exosomal miR-29 c may be a good indicator in patients with renal fibrosis.
