摘　　要：基于近50年临床、基础、理论、药学研究现代文献研究和项目组研究基础,动脉粥样硬化(AS)核心病因病机为"痰-瘀互损/共存",并具有不同发展阶段。在AS超早期主要是炎脂代谢紊乱、内皮功能障碍,应以"化痰和血、理气健脾"。在AS早中期主要是脂质条纹、斑块形成,应以"化瘀祛痰、健脾疏肝"。在AS中晚期主要是复合斑块、管腔狭窄,应以"补脾益肾、破瘀豁痰、搜风通络"。因而,中医药防治AS化瘀、祛痰、健脾贯穿始终,即"痰瘀论治、健脾为要"。On the basis of the last 5 decades literature review of clinical,basic,theoretical,pharmaceutical researches as well as our team research,the etiology and pathogenesis of atherosclerosis(AS)is phlegm-blood stasis mutual lesion/coexistence and has different stages and developments.In the super early stage of AS,there is mainly inflammation and lipid metabolism disorder as well as endothelial dysfunction.In this stage,the initial phlegm and blood stasis mutual junction should be treated with methods of eliminating phlegm and harmonizing the blood,regulating Qi and invigorating spleen.In the early-middle stage of AS,lipid streaks and plaques are mainly formed.We should remove phlegm and blood stasis,strengthen spleen and nourish liver.In the middle-late stage of AS,there is mainly complex plaque and narrow lumen,which is the mutual junction between phlegm and blood stasis,and we should use methods of strengthening spleen and kidney,eliminating blood stasis and phlegm,and dispelling wind to clear collaterals.Therefore,TCM prevention and treatment of AS should be removing phlegm and blood stasis,and strengthening the spleen,which should be throughout the whole process,namely treatment from phlegm and blood stasis,strengthening the spleen being the key.
