摘  要:目的观察人参总皂苷对非酒精性脂肪性肝病(NAFLD)大鼠胆汁酸代谢和胰岛素抵抗的影响。方法44只雄性Wistar大鼠随机分为普通饲料喂养的空白组(C,n=10)和高脂高糖饮食喂养的造模组(n=34),造模组8周后分为模型组(M)、西药组(R)、人参总皂苷组(RZ),分别给予生理盐水、罗格列酮、人参总皂苷灌胃治疗4周,同时继续高脂高糖饲料喂养。检测大鼠血清总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、丙氨酸氨基转移酶(ALT)、天门冬氨酸氨基转移酶(AST)、游离脂肪酸(FFA)和总胆汁酸(TBA),空腹血糖(FBG)、胰岛素(FINS)水平并计算胰岛素抵抗指数(IRI);取肝脏,做肝脏病理HE染色,观察光镜下脂肪变程度。结果RZ组大鼠血清TC、TG、FBG、FINS、IRI、FFA、TBA、ALT、AST水平较M组显著降低(P<0.05或P<0.01),HDL-C较M组显著升高(P<0.01),LDL-C水平无统计学意义(P>0.05)。光镜下观察见RZ组大鼠肝脂肪病变较M组明显减轻,而R组大鼠肝脂肪变性改善程度不明显。结论人参总皂苷能够显著降低NAFLD大鼠血清TC、TG、FFA、TBA、FBG和FINS的水平,改善胰岛素抵抗,改善肝功能,人参总皂苷对胆汁酸代谢及胰岛素抵抗的影响可能是其治疗NAFLD的主要机制。Objective To investigate the effects of ginsenosides medicines on bile acid metabolism and insulin resistance in nonalcoholic fatty liver disease rats(NAFLD).Methods Forty-four male rats were randomly divided into Control group(n=10)and Modeling group(n=34).We fed the rats of Modeling group with high-fat and high-sugar diet and others with standard chow for 8 weeks.Then the model rats were divided into model group(M),rosiglitazone group(R)and ginsenosides group(RZ).The M,R and RZ groups were still administered the same diet and given by physiological saline,rosiglitazone and ginsenosides medicines intragastric treatment 4 weeks,and then the levels of serum total cholesterol(TC),triglyceride(TG),low density lipid protein cholesterol(LDL-C),high density lipid protein cholesterol(HDL-C),alanineaminotransferase(ALT),aspartate amino transferase(AST),free fattyacid(FFA),total bile acid(TBA),fasting blood-glucose(FBG)and fasting insulin(FINS)were tested.And we figured out the insulin resistance index(IRI).The liver tissue was collected to be observed for liver histopathologic changes under light microscope.Results The levels of serum TC,TG,FBG,FINS,IRI,FFA,TBA,ALT and AST in the RZ group were lower than those of the M group(P<0.01,P<0.05).HDL-C was in the RZ grouphigher than that in the M group(P<0.01).The change of LDL-C was not obvious.Liver adipohepatic changes of the RZ group were reduced than those of the M group,but was not obvious difference in the R group.Conclusion The ginsenosides canevidently reduce the levels of serum TC,TG,FFA,TBA,FBG and FINS,and improve insulin resistance and liver function.Therefore,the effect of ginsenosides on bile acid metabolism and insulin resistance may be the main mechanism in the treatment of NAFLD.
