固本宣肺汤治疗稳定期慢性阻塞性肺疾病疗效及对免疫功能的影响

摘　　要：目的观察固本宣肺汤治疗稳定期慢性阻塞性肺疾病的临床疗效及对免疫功能的影响。方法选择2017年1月-2019年1月来医院治疗的慢性阻塞性肺疾病患者198例,依据随机对照法将患者分为研究组和对照组,每组99例。对照组患者采取沙美特罗替卡松粉吸入剂治疗,研究组在对照组治疗的基础上,采用固本利肺法治疗。3个月为1个疗程,连服2个疗程。比较两组中医症状分级量化表,治疗疗效,治疗前后6个月内急性加重次数,治疗前后肺功能指标FEV1、FVC和6 min步行距离,治疗前后免疫功能指标IgA、IgG和IgM水平和血清IL-8、α1-AT水平。结果研究组治疗总有效率为91.92%(91/99)高于对照组总有效率的80.81%(80/99)(P<0.05);研究组治疗后中医症候积分低于对照组(P<0.05);研究组治疗后发生急性加重次数低于对照组(P<0.05);研究组治疗后肺功能指标FEV1、FVC优于对照组,6 min步行距离较对照组明显增加(P<0.05);研究组治疗后免疫功能指标IgA、IgG和IgM水平明显优于治疗前,优于同期对照组(P<0.05);治疗组治疗后血清IL-8水平低于对照组,α1-AT水平高于对照组(P<0.05)。结论固本利肺结合沙美特罗替卡松粉吸入剂治疗稳定期慢性阻塞性肺疾病疗效确切,可有效的缓解患者的临床症状,抑制气道炎症反应,提高免疫球蛋白水平,增加运动耐力,提高治疗疗效,为临床提供了思路。Objective To observe theclinical efficacy of Guben Xuanfei Decoction in the treatment of stable chronic obstructive pulmonary disease and its effect on immune function.Methods A total of 198 patients with chronic obstructive pulmonary disease who were treated in our hospital from January 2017 to January 2019 were selected and divided into study group and control group according to randomized control method,with 99 cases in each group.The patients in the control group were treated with Salmeterol Fluticasone Powder for Inhalation.Those in the study group were treated with Guben Xuanfei Decoction on the basis of the treatment in the control group.Three months was a course of treatment,totally two courses of treatment.The therapeutic effect,the number of acute exacerbations within 6 months before and after treatment,FEV1,FVC and 6-minute walking distance before and after treatment,levels of IgA,IgG and IgM and serum levels of IL-8 andα1-AT were compared between the two groups.Results The total effective rate of the study group was 91.92%(91/99),higher than 80.81%(80/99)of the control group(P<0.05).The score of TCM symptoms in the study group was lower than that in the control group(P<0.05).The number of acute exacerbations in the study group was lower than that in the control group(P<0.05).The lung function indexes FEV1 and FVC in the study group were better than those in the control group after treatment.After treatment,the levels of IgA,IgG and IgM in the study group were significantly higher than those in the control group(P<0.05),and the levels of IL-8 in the treatment group were lower than those in the control group,and the levels ofα1-AT in the treatment group were higher than those in the control group(P<0.05).Conclusion Guben Xuanfei Decoctioncombined with Salmeterol Fluticasone Powder for Inhalation is effective in the treatment of stable chronic obstructive pulmonary disease.It can effectively relieve clinical symptoms,inhibit airway inflammation,improve the level of immunoglobulin,increase exercise endu
