清热败毒汤联合利巴韦林治疗小儿手足口病临床研究

摘　　要：目的观察清热败毒汤联合利巴韦林治疗小儿手足口病的临床治疗效果。方法将2018年7月-2019年5月收治的200例小儿手足口病患儿随机分为观察组与对照组,每组100例。对照组患儿均给予利巴韦林颗粒,观察组在对照组的基础上另口服给予清热败毒汤。治疗后,对两组患者的疗效进行评价。治疗期间,密切观察两组患儿总病程,疱疹恢复时间、口腔溃疡消退时间、体温恢复时间及食欲恢复时间。治疗前后,观察两组患儿的中医症状积分(疱疹,发热,咽红,流涎及神情倦怠)。治疗前后,分别检测两组患儿血清炎性因子含量,主要包括可溶性细胞间黏附分子-1(sICAM-1)、超敏C反应蛋白(hs-CRP)、血清干扰素-γ(IFN-γ)、白细胞介素-1β(IL-1β)及肿瘤坏死因子-α(TNF-α)。治疗期间,密切观察两组患儿药物不良反应发生情况,包括变态反应、肝脏损伤、肾脏损伤、头疼头晕及嗜睡等。结果治疗后,观察组总有效率显著高于对照组(P<0.05)。治疗期间,观察组总病程、疱疹恢复时间、口腔溃疡消退时间、体温恢复时间及食欲恢复时间均显著低于对照组(P<0.05)。治疗后,观察组疱疹,发热,咽红,流涎及神情倦怠评分均显著低于对照组(P<0.05)。治疗后,观察组患儿sICAM-1、hs-CRP、IFN-γ、IL-1β及TNF-α均显著低于对照组(P<0.05)。治疗期间,观察组患儿变态反应、肝脏损伤、肾脏损伤、头疼头晕、嗜睡等药物不良反应的发生率均明显低于对照组(P<0.05)。结论清热败毒汤联合利巴韦林治疗小儿手足口病疗效较好,可有改善患儿临床症状,缩短患儿患病时间,值得进行深入研究。Objective To investigate the effects of Qingre Baidu Decoction combined with ribavirin in the treatment of infantile hand-foot-mouth disease.Methods Patients(200 cases)with infantile hand-foot-mouth disease from July.2018 to May.2019 in our hospital were randomly divided into the control group(100 cases)and treatment group(100 cases).The control group received ribavirin,while patients in the treatment group were administered with Qingre Baidu Decoction on the basis of the control group.After treatment,the clinical effects were evaluated.During the treatment,the total course of disease,herpes recovery time,absolution time of oral ulcer,temperature recovery time and appetite recovery time were observed.Before and after treatment,the traditional Chinese medicine symptom scores(herpes,fever,pharynx,salivation and tiredness)of two groups were observed.Before and after treatment,serum inflammatory factors of the two groups were detected,including soluble intercellular adhesion molecular-1(sICAM-1),hypersensitive C-reactive protein(hs-CRP),serum interferon-γ(IFN-γ),interleukin-1β(IL-1β)and tumor necrosis factor-α(TNF-α).During the treatment,we observed two groups’occurrence of adverse drug reactions,including allergic,liver damage,kidney damage,such as headache,dizziness and lethargy.Results After treatment,the effective rate in treatment group was significantly higher than that of the control group(P<0.05).During the treatment,the total course of disease,herpes recovery time,absolution time of oral ulcer,temperature recovery time and appetite recovery time in the observation group were significantly shorter than those of the control group(P<0.05).After treatment,the symptom scores of herpes,fever,pharynx,salivation and tiredness in the observation group were significantly lower than those of the control group(P<0.05).After treatment,the levels of sICAM-1,hs-CRP,IFN-γ,IL-1βand TNF-αin the observation group were significantly lower than those of the control group(P<0.05).During the treatment,the rates of aller
