一站式复合手术在脑血管疾病治疗中的应用

摘　　要：目的探讨应用复合手术治疗脑血管疾病的安全性及有效性。方法回顾性分析一站式复合手术治疗的7例脑血管病患者,其中动静脉畸形3例,颈内巨大动脉瘤1例,硬脊膜动静脉瘘1例,颅内动脉瘤夹闭术2例。患者均接受一站式复合手术治疗,术中即刻CT检查术野情况,DSA造影评估病灶治疗效果。结果DSA提示血管畸形完全切除,瘘口完全闭塞,结扎颈内动脉后,颈动脉瘤未再显影,动脉瘤夹闭患者瘤体无残余,无死亡病例,切口感染,神经功能损伤等并发症。术中CT提示术野无渗血等。结论初步观察,应用一站式复合手术方式治疗脑血管病,安全、有效。Objective To investigate the safety and effectiveness of one-step surgery in the treatment of cerebrovascular diseases.Methods A retrospective analysis in 7 cases of cerebrovascular diseases treated by one-step surgery,including 3 cases of arteriovenous malformation,1 case of huge intracervical aneurysm,1 case of dural arteriovenous fistula,and 2 cases of intracranial aneurysm occlusion.All patients received one-step surgical treatment.Intraoperative CT examination was performed for assessment of the operative field,and DSA imaging was applied for assessing the treatment effect.Results DSA suggested complete resection of vascular malformation,complete occlusion of fistula,no redevelopment of carotid aneurysm after ligation of internal carotid artery,no residual aneurysm revealed after clamping.No death,wound infection,neurological impairment injury or other complications was recorded.CT suggested no bleeding in the operative field.Conclusion It is safe and effective to treat cerebrovascular disease by one-step surgery.
