摘 要:目的探讨急性荨麻疹患者血常规、C反应蛋白(CRP)、红细胞沉降率(ESR)的检测意义及相关性。方法对80例急性荨麻疹患者血常规、CRP、ESR进行回顾性分析,并与80例健康体检者进行比较,用Logistic回归分析进一步探讨急性荨麻疹患者相关变量,用受试者工作特征曲线(ROC曲线)确定诊断效能;对伴有系统症状和不伴系统症状的急性荨麻疹进行比较,用Logistic回归分析进一步探讨伴有系统症状的急性荨麻疹患者相关变量,用ROC曲线确定其预测效能。结果急性荨麻疹患者白细胞计数(WBC)、中性粒细胞计数(NEU)、淋巴细胞计数(LYM)、ESR、CRP较健康组升高(P均<0.05),且伴系统症状的急性荨麻疹患者CRP、ESR水平明显高于未伴系统症状的急性荨麻疹患者(P均<0.05)。Logistic回归分析显示,CRP、WBC是急性荨麻疹的危险因素;CRP是伴系统症状急性荨麻疹的独立危险因素。ROC曲线显示:CRP、WBC在急性荨麻疹诊断中的效能ROC曲线下面积(AUC)分别为0.853、0.968,CRP>WBC(P<0.05),CRP在急性荨麻疹的诊断效能较高;CRP预测急性荨麻疹伴系统症状效能最高,最佳截点为10.85,其敏感度为90.0%,特异度为76.7%。结论CRP与急性荨麻疹的关系密切,尤其对伴系统症状的急性荨麻疹可能有预测价值。Objective To investigate the relationship of laboratory data with acute urticaria.Methods Laboratory data including routine blood test,C-reactive protein(CRP),erythrocyte sedimentation rate(ESR) were collected from 80 patients with acute urticaria and 80 healthy controls,and analyzed retrospectively.Logistic regression analysis were used to determine variables associated with acute urticaria patients or(and)the partial patients of combining systems.The receiver operating characteristic(ROC)curve were applied to estimate the predictive efficacy of these parameters.Results WBC,NEU,LYM,ESR,CRP were all significantly higher in the patients with acute urticaria than in healthy controls(all P<0.05),and CRP,ESR were both notable higher in acute urticaria patients combining systemic symptoms than in those without(both P<0.05).Logistic regression analysis showed that CRP,WBC were risk factors for acute urticaria patients(P<0.05).CRP was an independent risk factor for the patients with acute urticaria of combining symptoms.The ROC curve analysis revealed that the area under ROC curve(AUC)of CRP and WBC were 0.853 and 0.968,CRP was higher than WBC(P<0.05);the optimal cut-off point of CRP was 10.85 in the patients with acute urticaria of combining systems,with the sensitivity and specificity of 90.0%and 76.7% respectively.Conclusion The relationship between CRP and acute urticaria was more closely,especially in those with acute urticaria of combining systems.CRP is a potential predictor for acute urticaria of combining systems.
