舒筋活血祛痛方熏蒸配合电针夹脊穴治疗L4~5腰椎间盘突出症合并骶髂关节功能紊乱症疗效观察

摘　　要：目的观察舒筋活血祛痛方熏蒸配合电针夹脊穴治疗L4~5腰椎间盘突出症(LDH)合并骶髂关节功能紊乱症的临床疗效。方法将2017年1月—2019年7月亳州市中医院诊治的81例L4~5 LDH合并骶髂关节功能紊乱症患者随机分为对照组40例与观察组41例,对照组给予常规治疗加普通针刺,观察组在常规治疗基础上给予舒筋活血祛痛方熏蒸配合电针夹脊穴治疗,2组疗程均为4周。比较2组临床疗效及干预前后疼痛视觉模拟评分(VAS)、日本骨科学会(JOA)腰功能评分、腰椎活动度、Oswestry功能障碍指数评分(ODI)。结果观察组总有效率为90.2%(37/41),显著高于对照组的70.0%(28/40),差异有统计学意义(P<0.05)。与治疗前比较,治疗后2组VAS评分、ODI评分均明显降低(P均<0.05),JOA评分(主观症状、临床症状、日常生活能力、总评分)明显增加(P均<0.05),且治疗后观察组以上评分与对照组比较差异均有统计学意义(P均<0.05)。与治疗前比较,治疗后2组腰椎活动度(前屈、后伸、左旋、右旋、右侧屈、左侧屈)均明显增加(P均<0.05),且治疗后观察组上述指标均明显高于对照组(P均<0.05)。结论舒筋活血祛痛方熏蒸配合电针夹脊穴治疗L4~5 LDH合并骶髂关节功能紊乱症疗效优于普通针刺治疗,可明显缓解疼痛症状,改善腰椎活动度与功能,改善日常生活质量。Objective It is to observe the clinical efficacy of Shujin Huoxue Qutong decoction fumigation combined with electroacupuncture at Jiaji points in the treatment of L4~5 lumbar disc herniation(LDH)with sacroiliac joint dysfunction.Methods 81 patients with L4~5 LDH combined with sacroiliac joint dysfunction who were diagnosed and treated by Bozhou Hospital of Traditional Chinese Medicine from October 2017 to July 2019 were randomly divided into a control group of 40 cases and an observation group of 41 cases.The control group was given conventional treatment plus ordinary acupuncture,the observation group was given Shujin Huoxue Qutong decoction fumigation combined with electroacupuncture at Jiaji points on the basis of the conventional treatment,and the course of treatment was 4 weeks.The clinical efficacy and visual analogue scale(VAS)of pain,JOA lumbar function score,lumbar mobility,and Oswestry dysfunction index(ODI)scores before and after intervention were compared between the two groups.Results The total effective rate of the observation group was 90.2%(37/41),which was significantly higher than that 70.0%(28/40)of the control group(P<0.05).Compared with before treatment,the VAS score and ODI score of the two groups were significantly reduced after treatment(P<0.05),JOA score(subjective symptoms,clinical symptoms,activities of daily living,total score)were significantly increased(P<0.05),the improvement of the above scores in the observation group was better than that of the control group(P<0.05).Compared with before treatment,the range of motion of the lumbar spine(forward flexion,extension,left rotation,right rotation,right flexion,left flexion)were increased significantly after treatment in the two groups(P<0.05),and the above indicators in the observation group after treatment were obvious higher than those in the control group(P<0.05).Conclusion Shujin Huoxue Qutong decoction fumigation combined with electroacupuncture at Jiaji points is more effective than conventional treatment in the treatment of L4~5
