摘 要:在颈椎退行性病变的治疗中,颈椎前路椎间盘切除植骨融合内固定术和颈椎前路椎体次全切除植骨融合内固定术可以彻底解除神经与脊髓压迫,重建颈椎稳定性,促进节段融合,取得了良好的临床效果。钛网笼(TMCs)因拥有较好的生物力学稳定性,使大多数患者在术后能够取得良好的治疗效果,因此成为被采用最多的内置物。然而,内置物的沉降问题普遍存在且一直未解决,沉降的发生与接触面积、骨质及终板强度、手术操作等有紧密联系,近年来通过改善TMCs结构、优化手术操作等方式,使沉降发生的可能性进一步降低。In the treatment of cervical degenerative diseases,anterior cervical surgery such as anterior cervical discectomy and fusion and anterior cervical corpectomy and fusion,can thoroughly remove nerve and spinal cord compression,reconstruct the cervical stability,and promote the segmental fusion,which has obtained good clinical effect.Titanium mesh cages(TMCs)are featured with good biomechanical stability,which enable good effect in most of the patients after surgery,making it the mostly adopted implants in the recent 20 years.However,the subsidence of the implant is a widespread problem and hasn′t been solved yet.The occurrence of subsidence is related to contact area,bone and endplate strength,and operation method.In recent years,the possibility of subsidence has been further reduced by improving the structure of TMCs and optimizing the operative procedures.
