摘 要:目的探究糖尿病周围神经病变(DPN)患者角膜神经参数变化及临床意义。方法回顾性选取该院2017年2月—2019年2月收治的糖尿病患者240例,其中单纯糖尿病患者有130例,为A组;DPN患者110例(分期:Ⅰ期35例,Ⅱ期49例,Ⅲ期26例),为B组;将同期入院体检的110名健康志愿者分为对照组,对比分析3组及DPN不同分期患者的角膜神经参数。结果相比对照组与A组,B组角膜神经纤维密度(CNFD)、角膜神经分支密度(CNBD)、角膜神经纤维长度(CNFL)均有降低,神经纤维弯曲度(CNFT)有所上升,差异有统计学意义(P<0.05);相比Ⅰ期与Ⅱ期,Ⅲ期DPN患者CNFD、CNBD、CNFL明显降低,CNFT明显上升,差异有统计学意义(P<0.05);DPN分期与CNFL、CNFD、CNBD负相关(P<0.05)。结论角膜神经参数与DPN发展密切相关,可作为评估DPN患者病情严重程度的重要指标。Objective To investigate the changes and clinical significance of corneal nerve parameters in patients with diabetic peripheral neuropathy(DPN). Methods A retrospective selection of 240 diabetic patients admitted to our hospital from February 2017 to February 2019, including 130 patients with simple diabetes, group A;110 patients with DPN(stage:35 cases in stage Ⅰ, 49 cases in stage Ⅱ), 26 patients in stage Ⅲ), group B;110 healthy volunteers who were admitted to the hospital during the same period were divided into control groups, and the corneal nerve parameters of the three groups and patients with different stages of DPN were compared and analyzed. Results Compared with control group and group A,group B corneal nerve fiber density(CNFD), corneal nerve branch density(CNBD), corneal nerve fiber length(CNFL) all decreased, and nerve fiber curvature(CNFT) increased, the difference Significantly statistically significant(P<0.05);CNFD,CNBD, and CNFL of patients with stage DPN were significantly lower than those of stage I and stage II, and CNFT was significantly increased. The difference was statistically significant(P<0.05);CNFD and CNBD were negatively correlated(P<0.05). Conclusion Corneal nerve parameters are closely related to the dev elopment of DPN, and can be used as an important indicator to evaluate the severity of DPN patients.
