血清尿酸水平与充血性心力衰竭严重程度的相关性分析

摘　　要：目的考察血清尿酸水平与充血性心力衰竭严重程度的相关性。方法方便选择2013年9月—2016年9月该院心内科收治的充血性心力衰竭患者236例,根据纽约健康协会(NYHA)对患者心功能进行分级。另外,选取与充血性心力衰竭患者同一阶段住院且心功能正常的患者128例作为对照组,检测充血性心理衰竭患者组和对照组患者的血清尿酸水平。结果NYHA Ⅳ级高尿酸血症发生率为62.7%高于NYHA Ⅲ级38.5%、NYHA Ⅱ级17.8%,差异有统计学意义(F=29.596,P<0.05);NYHA Ⅳ级血清尿酸浓度为(538.06±59.1)μmol/L高于NYHA Ⅲ级(418.99±55.6)μmol/L、(315.26±57.3)μmol/L,差异有统计学意义(F=13.238,P<0.05);NYHA Ⅳ级LVEF为(31.2±3.5)%低于NYHA Ⅲ级(40.1±2.3)%、NYHA Ⅱ级(48.8±3.7)%,差异有统计学意义(F=26.040,P<0.05);NYHA Ⅳ级LVEDd(65.3±5.7)mm高于NYHA Ⅲ级(58.4±6.1)mm、NYHA Ⅱ级(47.3±5.2)mm,差异有统计学意义(F=8.983,P<0.05)。结论血清尿酸水平越高,充血性心力衰竭越严重。Objective To investigate the correlation between serum uric acid level and the severity of congestive heart failure.Methods From September 2013 to September 2016,236 patients with congestive heart failure were admitted to the department of cardiology,and the heart function was graded according to the New York Health Association(NYHA)score.In addition,128 patients who were hospitalized at the same stage as patients with congestive heart failure and had normal heart function were conveniently selected as the control group,and the serum uric acid levels in the patients with congestive psychological failure group and the control group were detected.Results The incidence of NYHA Ⅳ hyperuricemia was 62.7%higher than NYHA Ⅲ 38.5%and NYHA Ⅱ 17.8%,the difference was statistically significant(F=29.596,P<0.05);NYHA Ⅳ serum uric acid concentration was(538.06±59.1)μmol/L higher than NYHA Ⅲ(418.99±55.6)μmol/L,(315.26±57.3)μmol/L,the difference was statistically significant(F=13.238,P<0.05);NYHA The level Ⅳ LVEF was(31.2±3.5)%lower than NYHA level Ⅲ(40.1±2.3)%,NYHA level Ⅱ(48.8±3.7)%,,the difference was statistically significant(F=26.040,P<0.05).LVEDd(65.3±5.7)mm of NYHA grade Ⅳ was higher than that of NYHA grade Ⅲ(58.4±6.1)mm,NYHA grade Ⅱ(47.3±5.2)mm,the difference was statistically significant(F=8.983,P<0.05).Conclusion The higher the serum uric acid level,the more severe the congestive heart failure.
