髓心减压联合补肾活血方治疗股骨头坏死疗效观察

摘　　要：目的:观察髓心减压术联合自拟补肾活血方治疗早期股骨头坏死的临床疗效。方法:2015年8月-2018年9月,将40例ARCOⅠ-Ⅱ期股骨头坏死患者按入院顺序随机分为2组。其中观察组20例,采用髓心减压术联合自拟补肾活血方治疗。对照组20例,仅给予髓心减压术治疗。记录两组患者术前VAS评分、Harris评分,术后1年VAS评分、Harris评分,观察术后1年影像学进展情况,计算改善率。结果:所有患者术后均未发生血栓、感染等严重并发症,组内比较,2组患者术后1年VAS评分、Harris评分与术前VAS评分、Harris评分比较均有差异(P<0.05),组间比较,2组术后1年VAS评分比较无差异(P>0.05),2组术后1年Harris评分比较有差异(P<0.05),并且术后1年影像学进展比较,观察组改善率为80%,高于对照组的70%。结论:髓心减压术联合自拟补肾活血方治疗ARCOⅠ-Ⅱ期股骨头坏死疗效确切,能够改善髋关节功能,延缓股骨头坏死的进展,是一种较好的保头治疗方法。Objective:To evaluate the clinical effects of core decompression with Bushen Huoxue recipe for osteonecrosis of femoral head.Methods:From August 2015 to September 2018,40 patients with ARCOⅠ-Ⅱosteonecrosis of femoral head were studied.Twenty cases were in the observation group and 20 cases were in the control group.The observation group adopted core decompression with Bushen Huoxue recipe,while the control group adopted core decompression.The two groups were compared before and after the surgery by Harris scores,VAS scores and imaging changes.Results:There were no complications of thrombus and infection within 12 months after surgery.Compared to the data before the treatment,VAS scores and Harris scores at one-year follow-up in both groups showed better outcomes(P<0.05).The VAS scores of the observation group was lower than that of the control group,but there was no significant difference between them(P>0.05),Harris score was better than the control group,with statistically significant difference(P<0.05).The rate of necrosis of femoral head in the observation group was 80%higher than 70%in the control group.Conclusion:The effect of core decompression with Bushen Huoxue recipe in treatment of osteonecrosis of femoral head is accurate,can delay the progress of femoral head necrosis and the improvement of hip function is obvious,which is a better means of head preserving treatment.
