摘  要:目的探讨学龄痉挛型脑瘫儿童汉语普通话声调发音的特点,为临床治疗提供依据。方法采用发声系统测量仪(Dr.Speechvocal measurement)对41例6~16岁痉挛型脑瘫儿童(脑瘫组)与35例6~16岁正常儿童(对照组)的"蛙(wā)、娃(wá)、瓦(wǎ)、袜(wà)"(选用零声母/w/,为了排除声韵组合对声调的影响)调值特征进行分析和比较。结果 (1)与正常组相比,脑瘫组儿童一声调的起点值(Z=-2.225,P=0.026)和终点值(Z=-2.162,P=0.031)均较低,而一声调斜率两组之间差异无统计学意义(P>0.05)。(2)脑瘫组儿童三声调控制最差,错误率达46.34%,其中有9例三声调的基频值杂乱无序,5例基频值呈上升趋势(类似二声调),3例基频值呈下降趋势(类似四声调),2例基频值为先升后降,因此排除以上19例,以其余22名脑瘫儿童的"瓦(wǎ)"调值与正常组进行比较,显示两组起点值(Z=-0.041,P=0.918)、折点值(Z=-0.103,P=0.918)、终点值(Z=-0.415,P=0.679)、斜率k降值(Z=-0.383,P=0.702)、斜率k升值(Z=-0.217,P=0.828)差异均无统计学意义。(3)脑瘫组二声调和四声调的起点值、终点值和斜率与正常组差异均无统计学意义。结论本组学龄痉挛型脑瘫儿童音调发音中一声调斜率正常,但起点和终点的调值较低;三声调的错误率最高,表现为声调控制能力较差;二声调和四声调较容易掌握;临床治疗中,应注意提高痉挛性脑瘫儿童一声调的调值和三声调的控制能力。Objective To explore the characteristics of tonal pronunciation of Chinese Mandarin in children with cerebral palsy, and to provide evidence for clinical therapy.Methods Using the Dr.Speech vocal measurement to anslysis the tone pronunciation of 41 subjects of 6~16 year old cerebral palsy children and 35 subjects of 6~16 year old normal children. The test corpus was "wā, wá, wǎ, and wà"(select zero initials/w/, in order to eliminate the influence of the combination of sound and rhyme on the tone).Results(1)Compared with the normal children, children with spastic cerebral palsy had a lower high-level tone’s starting point(Z=-2.225, P=0.026) and ending point(Z=-2.162, P=0.031), but these was no statistically significant difference in the slope of nigh-level tone between the two groups.(2)The falling-rising tone’s pronunciation of children with spastic cerebral palsy was the worst, and with an error rate of 46.34%. Among them, 9 subjects had irregular changes of the pitch values, 5 subjects’ pitch values showed an upward trend. Similar to the high-rising tone, 3 subjects’ pitch values showed a downward trend(similar to the and high-falling tone), and 2 subjects’ pitch values showed up first and then decreased. So the above data could not be used, and the remaining 22 cerebral palsy children’s "wǎ" were used to analysis. Comparison with typical children, these children’s starting point values(Z=-0.041, P=0.918), lowest point values(Z=-0.103, P=0.918), ending point values(Z=-0.415, P=0.679), the decreasing slope(Z=-0.383, P=0.702) and rising slope(Z=-0.217, P=0.828) of falling-rising tone were not significantly different.(3)There were no significant differences in the starting point, ending point and slope of high-rising and high-falling tones between cerebral palsy children and typical children.Conclusion(1)The F0 of the starting and ending points of children with spastic cerebral palsy are significantly lower than that of typical children, and the F0 is lower.(2)The children with spastic cerebral
