从《当代名医临证精华痹证专辑》看痹证的辨证施治  

Discussion on the Treatment Based on Syndrome Differentiation of Bi Disease from the Monography on Arthralgia Syndrome that is the Clinical Essence of Contemporary Famous doctors

摘　　要：通过对《当代名医临证精华痹证专辑》中医论医案的统计分析,总结归纳了痹证的辨证分型与用药特色。寒湿侵袭、火热炽盛、瘀血阻滞3个证型较多,基本与中医内科学分型一致,但其中的风热侵袭、风热血瘀、卫气不和3个证型内科学中没有提及;在用药统计上,用药频次前10位药分别是桂枝、当归、白芍、甘草、牛膝、防己、黄芪、薏仁、羌活、生地黄。在用药经验上,朱良春的虫类药运用、董建华的药对、谢海洲对疼痛的用药、焦树德等医家的治疗用大剂、张沛虬的酒剂、汪世英等医家的外用制剂等都颇具特色。Analyzing medical cases from Monograph on bi disease that is the clinical essence of contemporary famous doctors,we summarize syndrome classification and medication characteristic of arthralgia syndrome.Three main patterns of arthralgia syndrome are cold and damp invasion,fire and heat exuberance and static blood obstruction,that are consistent with the classification of internal medicine of traditional Chinese medicine.However,there are three patterns of arthralgia syndrome not mentioned by internal medicine of traditional Chinese medicine,wind and heat invasion,wind-heat blood stasis and defensive qi disharmony.According to drug application statistics,there are top 10 drugs with frequent clinical application,Cassia Twig,Chinese Angelica,Radix Paeoniae Alba,Licorice,Achyranthes bidentata,Stephania Tetrandrae,Astragalus,Coix seed,Notopterygium and Rehmannia root.From drug application experience,famous doctors have their own unique features,such as insect drugs application of Dr.ZHU Liangchun,medicine suits of Prof.Dong Jianhua,drugs for pain of Dr.XIE Haizhou,treament with high doses of drugs of Prof.JIAO Shude and another doctors,medicinal liquor of Dr.ZHANG Peiqi,and the external preparation of Dr.WANG Shiying and another doctors.
