柴胡达原饮治疗脾胃湿热型功能性消化不良随机对照试验

摘　　要：目的观察柴胡达原饮加减治疗脾胃湿热型功能性消化不良(FD)患者的临床疗效和安全性。方法使用计算机随机,将门诊收治的72例脾胃湿热型功能性消化不良患者随机分为2组(n=36)。对照组给予枸橼酸莫沙必利片,治疗组予以柴胡达原饮加减口服。治疗1个月后,比较2组患者药物的治疗总有效率、中医症状积分以及血浆胆囊收缩素、血管活性肠肽、胃动素、胃泌素的变化和不良反应情况。结果治疗组总有效率为83.33%,对照组总有效率为77.78%,2组比较无统计学差异(P>0.05)。治疗组中医症状积分均较治疗前明显降低(P<0.01),对照组除头身困重、口苦口黏和小便短黄症状外,其他症状积分均较治疗前明显降低(P<0.01)。治疗组和对照组血浆胆囊收缩素、血管活性肠肽水平均较治疗前明显降低,血浆胃动素和胃泌素水平均明显升高(P<0.01),2组比较无统计学差异(P>0.05)。结论柴胡达原饮加减可有效改善脾胃湿热型功能性消化不良患者的临床症状,可以调节功能性消化不良患者胃肠激素分泌,在改善中医症状方面较西药枸橼酸莫沙必利片更具优势,且无不良反应。Objective Investigate the clinical efficacy and safety of Chaihu Dayuan Decoction in the treatment of functional dyspepsia with pattern of dampness-heat in stomach and spleen.Methods 72 patients of functional dyspepsia with pattern of dampness-heat in stomach and spleen were randomly divided into treatment group (n=36) and control group (n=36) according to computer random method.The control group was treated with Mosapride Citrate Tablets and the treatment group was treated with Chaihu Dayuan Decoction.After a month of treatment,the efficiency rate,TCM symptom scores,the levels of Plasma Cholecystokinin,Vasoactive intestinal peptide,Motilin and Gastrin,and adverse reactions of the patients were compared between the two groups.Results The efficiency rate of treatment group was 83.33%,that of control group was 77.78%.There was no significant difference in two groups (P>0.05).The TCM symptom scores in treatment group were significantly decreased after treatment (P<0.01),Which in control group except for heavy head and body,bitter and glutinous in mouth,and short yellow urine were samely decreased (P<0.01).The levels of Plasma Cholecystokinin and Vasoactive intestinal peptide of the patients were significantly decreased,and that of Motilin and Gastrin were increased in two group (P<0.01).There was no significant difference between two groups (P>0.05).Conclusion Chaihu Dayuan Decoction can effectively alleviate the clinical symptoms of patients with functional dyspepsia with pattern of dampness-heat in stomach and spleen,and can regulate the secretion of gastrointestinal hormone.Compared with Mosapride Citrate Tablets,it has more advantage in improving TCM symptoms,and no adverse effects.
