摘　　要：目的探讨曲度牵引配合推拿疗法治疗颈型颈椎病的临床疗效及作用机制。方法选取2018年3月至2019年3月就诊的颈型颈椎病患者84例,采用随机数字表法分为观察组42例和对照组42例。对照组采用传统坐位直线牵引配合中频电治疗,观察组采用卧位曲度牵引配合推拿疗法,连续治疗2个疗程共20天。治疗前后摄颈椎正侧位片,测量颈椎曲度(Borden氏测量法),测定NDI指数及VAS评分,并检测血清炎症因子TNF-α、IL-6、hs-CRP水平,综合评价临床疗效及作用机制。结果治疗两个疗程后,观察组颈椎曲度较治疗前及对照组明显增加(P<0.05),NDI指数及VAS评分较治疗前及对照组明显降低(P<0.05);治疗后,观察组血清TNF-α、IL-6水平明显下降,与治疗前及对照组相比差异具有显著性(P<0.05),血清hs-CRP水平与治疗前具有显著性差异(P<0.05),但和观察组相比无显著性差异(P>0.05)。结论曲度牵引配合推拿疗法能显著改善颈型颈椎病患者的颈椎生理曲度、NDI指数及VAS评分,其作用机制与重建颈椎生物力学平衡、改善机体炎症状态有关。Objective To observe the effect of curvature traction combined with massage on cervical spondylosis of cervical type and explore the mechanism. Methods 84 patients with cervical spondylosis of cervical type were enrolled, and were randomly divided into two groups, 42 in each group. The control group was treated with Linear traction combined with intermediate frequency electrotherapy, and the observation group was given curvature traction combined with massage. Positive and lateral radiograph of the cervical spine, cervical curvature, Neck Disability index, Visual analogue scale, Inflammatory factors includingTNF-α, IL-6, hs-CRP were assessed before and 20 days after treatment. Results After 20 days of treatment, cervical curvature in observation group was significantly higher than that before treatment and control group(P<0.05), and the scores of NDI, along with VAS, was significantly lower than that before treatment and control group(P<0.05). There was also significant difference in the cervical curvature, NDI and VAS in the control group before and after treatment(P <0.05). After 20 days of treatment,the level of the serum TNF-α and IL-6 in observation group was significantly lower than that before treatment and control group(P<0.05). The level of the serum hs-CRP in observation group was significantly lower than that before treatment, but there was no significant difference between two groups after treatment(P>0.05). Conclusion curvature traction combined with massage can significantly improve cervical curvature、NDI and VAS in Cervical spondylosis of cervical type patients, the mechanism in which is considered to corelate with the reconstruction of biomechanical balance of cervical vertebra and the improvement of inflammatory state.
