摘 要:目的探讨流入翻转恢复(IFIR)非对比剂增强肾动脉MRA在临床中的应用价值。方法选取2017年11月—2019年11月期间进行IFIR非对比剂增强肾动脉MRA的患者37例,进行肾动脉多层螺旋CT血管成像(MSCTA)检查,对比分析两者之间肾动脉的检出率及狭窄程度,并判断IFIR非对比剂增强肾动脉MRA序列在临床应用中的优、劣势。结果37例受检者中清晰显示肾动脉83支(含副肾动脉9支)在MSCTA均显示清晰,经IFIR序列检出肾动脉81支(含副肾动脉7支),检出率为97.6%。其中MSCTA检查出肾动脉狭窄(RSA)22例,IFIR序列检查出RSA 23例,经IFIR序列检查的患者表现为肾动脉不同程度狭窄32支,而接受MSCTA肾动脉检查肾动脉狭窄33支。两者之间一致性无统计学差异。结论IFIR序列与MSCTA比较能较好显示肾动脉狭窄,敏感性及准确性高,可作为肾性高血压或肾动脉狭窄患者首选的筛查手段。Objective To investigate the clinical value of non contrast enhancement of renal artery MRA by inflow reversal recovery(IFIR).Methods From November 2017 to November 2019,37 patients with renal artery MRA enhanced by IFIR non contrast agent were selected for MSCTA examination of renal artery.The detection rate and stenosis degree of renal artery between them were compared and analyzed.The advantages and disadvantages of IFIR non contrast agent MRA sequence in clinical application were judged.Results In 37 cases,83 renal arteries(including 9 accessory renal arteries)were clearly displayed on MSCTA.Eighty-one renal arteries(including 7 accessory renal arteries)were detected by ifir sequence.The detection rate was 97.6%.Among them,22 cases of renal artery stenosis(RSA)were detected by MSCTA,and 23 cases were detected by ifir sequence.Thirty-two cases showed different degrees of renal artery stenosis by ifir sequence,and 33 cases underwent MSCTA renal artery examination.There was no significant difference between the two.Conclusion Compared with MSCTA,IFIR sequence can show renal artery stenosis with high sensitivity and accuracy.It can be used as the first screening method for patients with renal hypertension or renal artery stenosis.
