摘　　要：目的:探讨肿瘤坏死因子-α(TNF-α)在口腔鳞癌组织中的表达及临床意义。方法:取78例口腔鳞癌手术患者的癌组织与癌旁组织(距离癌组织≥3 cm)标本,采用免疫组织化学法测定TNF-α表达,收集患者性别、年龄、肿瘤直径、病理类型、分化程度、分期及淋巴结转移等临床病理资料、分析不同临床病理状态下TNF-α的表达差异,采用Logistic分析TNF-α表达的影响因素。结果:癌旁组织中TNF-α阳性细胞表达较少或无表达,癌组织中TNF-α阳性表达于细胞的胞质、包膜亦有少量表达、细胞核内无表达;癌组织TNF-α阳性表达率57.69%(45/78)高于癌旁组织12.82%(10/78),差异有统计学意义(χ^2=6.942,P=0.025);癌组织中TNF-α的阳性表达率与肿瘤直径、分化程度、分期及淋巴结转移差异有统计学意义(P<0.05);多因素Logistic分析结果表明,肿瘤直径、分化程度、分期及淋巴结转移是口腔鳞癌组织TNF-α阳性表达的影响因素(P<0.05)。结论:口腔鳞癌组织中TNF-α呈高表达,可反映患者疾病严重程度,有望成为口腔鳞癌治疗新靶点。Objective:To investigate the expression and clinical significance of tumor necrosis factor-α(TNF-α)in oral squamous cell carcinoma.Methods:78 patients with oral squamous cell carcinoma and adjacent tissues(≥3 cm from the lesion)were used as specimens.Immunohistochemical method was used to determine the positive expression of TNF-α in different tissues.Clinical information(gender,age,tumor diameter and pathology type,degree of differentiation,tumor stage and lymph node metastasis and other clinicopathological conditions)was collected.Then,analyzing the expression differences of TNF-α under different clinicopathological conditions.Logistic analysis was used to analyze the influencing factors of TNF-α expression.Results:TNF-α-positive cells showed less or no expression in adjacent tissues;TNF-αwas positively expressed in the cytoplasm of the cancer tissues,a few expression in capsule,and there was no expression in the nucleus.The positive expression rate of TNF-α in cancer tissue was 57.69%(45/78),which was higher than that in adjacent tissue(12.82%,10/78),the difference was statistically significant(χ^2=6.942,P=0.025);The differences between positive expression rate of TNF-α in cancer tissue and tumor diameter,degree of differentiation,tumor stage,and lymph node metastasis were statistically significant(P<0.05).The results of multivariate Logistic analysis showed that tumor diameter,differentiation degree,tumor stage,and lymph node metastasis were the influential factors of positive expression of TNF-α in oral squamous cell carcinoma(P<0.05).Conclusion:High expression of TNF-α in oral squamous cell carcinoma tissues can reflect the severity of patients'disease,and it is expected to become a new target for the treatment of oral squamous cell carcinoma.
