颈部推拿联合糖皮质激素注射治疗先天性斜颈临床研究

摘　　要：目的探讨颈部推拿联合糖皮质激素疗法与单纯颈部推拿疗法对先天性斜颈(CMT)疗效的差异。方法选取2016年10月至2018年4月该院收治的CMT患儿80例,采用随机数字表法分为治疗组和对照组,每组40例。治疗组给予颈部推拿联合糖皮质激素注射治疗,对照组采用单纯颈部推拿治疗。观察2组患儿治疗前后患侧健侧胸锁乳突肌厚度(R)、健侧患侧胸锁乳突肌长度(D)的变化,对比颈部活动度、头部倾斜度、面部畸形度、肌肉紧张度及彩色多普勒超声检查结果,并对其有效率进行评估。结果治疗组患儿总有效率明显高于对照组,差异有统计学意义(P<0.05)。治疗组患儿治疗后R、D明显低于对照组,差异均有统计学意义(P<0.05)。2组患儿治疗后颈部活动度、头部倾斜度、面部畸形度、肌肉紧张度及彩色多普勒超声检查结果评分均较治疗前明显改善,且治疗组患儿治疗后颈部活动度、头部倾斜度、面部畸形度、肌肉紧张度及彩色多普勒超声检查结果评分及其治疗前后评分差值均较对照组改善明显,差异均有统计学意义(P<0.05)。结论颈部推拿联合糖皮质激素局部注射治疗CMT疗效显著,值得临床推广应用。Objective To investigate the difference in the effects between the cervical massage combined with glucocorticoid injection therapy and simple neck massage therapy in treating congenital muscular torticollis(CMT).Methods Eighty children cases of CMT in this hospital from October 2016 to April 2018 were selected and divided into the control group and treatment group by adopting the random number table method,40 cases in each group.The treatment group was given the cervical massage combined with glucocorticoid injection therapy,while the control group adopted the simple neck massage therapy.The ratios of the thickness(R)and length(D)between the healthy side and sick side cleidomastoid;the neck activity,head tilt,facial deformity,muscle tension and color Doppler ultrasound examination results were observed in the two groups,and the effective rate was evaluated.Results The total effective rates in the treatment group was significantly higher than that in the control group,and the difference between the two groups was statistically significant(P<0.05).R and D after treatment in the treatment group were significantly lower than those in the control group,and the differences between the two groups were statistically significant(P<0.05).The neck mobility,head tilt degree,facial deformity,muscle tension,and color Doppler ultrasound examination results after treatment in the two groups were significantly improved compared with those before treatment,moreover the neck mobility,head tilt degree,facial deformity,muscle tension,color Doppler ultrasound examination results after treatment and the difference of the scores before and after treatment in the treatment group were significantly improved compared with those in the control group,and the differences between the two groups were statistically significant(P<0.05).Conclusion The cervical massage combined with glucocorticoid local injection therapy in treating CMT has significant effect and is worthy for clinical promotion and application.
