酒精性肝病患者瘀血舌象与肝功能指标的相关性研究

摘　　要：目的:探讨酒精性肝病患者瘀血舌象与肝功能指标变化的相关性。方法:纳入就诊于宁波市中医院有瘀血舌象的酒精性肝病患者150例,根据舌象分成紫舌、绛舌及青舌组;统计不同瘀血舌象在酒精性肝病不同分型中的分布。检测所有患者的天冬氨酸氨基转移酶(AST)、丙氨酸氨基转氨酶(ALT)、白蛋白(A)、球蛋白(G)、总胆红素(TBIL)、直接胆红素(DBIL)、间接胆红素(IBIL)等指标;分析不同瘀血舌象与上述指标变化的相关性。结果:绛舌组患者的总胆红素、直接胆红素、碱性磷酸酶指标明显低于紫舌组和青舌组;而绛舌组的白蛋白、前白蛋白、白球比例、天冬氨酸氨基转移酶、胆碱酯酶明显高于紫舌组和青舌组。舌象瘀血程度与总胆红素、直接胆红素、天冬氨酸氨基转移酶、碱性磷酸酶呈明显正相关,而与白蛋白、前白蛋白、白球比例、胆碱酯酶呈明显的负相关。结论:酒精性肝病的瘀血舌象与患者肝功能指标变化具有一定的相关性,随着瘀血程度的加深,肝脏功能就越差,预后越差。Objective:To investigate the relationship of blood stasis tongue image and liver function indexes in patients with alcoholic liver disease.Methods:150 Cases with alcoholic liver disease admitted to Ningbo hospital of Chinese medicine were enrolled and divided into purple tongue group,purple-red group and green tongue group according to tongue image,the distribution of above three kinds of blood stasis tongue image in each type of ALD was censused;Blood liver liver function indexes such as AST,ALT,albumin,globulin,TBIL,DBIL,IBIL,etc.were determined,the correlation between blood stasis tongue image and liver function indexes changes was analyzed.Results:TBIL,DBIL,and ALP levels in purple-red group were significantly lower than those in purple tongue group and green tongue group,the albumin,PA,A/G,AST and CHE levels in purple-red group were significantly higher than those in purple tongue group and green tongue group.The extent of tongue purpura was positively correlated with TP,DBIL,AST and ALP,but was negatively correlated with albumin,PA,A/G,AST and CHE.Conclusion:There is a certain correlation between the blood stasis tongue image and liver function indexes in patients with alcoholic liver disease,with the deepening of the degree of tongue purpura,liver function and the prognosis were worse.
