加味解毒清火合剂治疗小儿手足口病的疗效观察及其对炎性细胞因子的影响

摘　　要：目的:探讨加味解毒清火合剂治疗小儿手足口病湿热蒸盛型的临床疗效及对炎性细胞因子IL-6(白介素-6)、IL-10(白介素-10)、TNF-α(肿瘤坏死因子-α)的影响。方法:60例湿热蒸盛型手足口病患儿按随机分为中药组和西药组各30例,中药组予加味解毒清火合剂口服治疗,西药组予利巴韦林注射液静滴治疗;连续用药3~6 d,评价临床疗效;治疗前后观察主要症状发热、皮疹、口腔粘膜疱疹的改善情况,采用ELISA方法测得血清中炎性因子IL-6、IL-10、TNF-α含量,比较变化的意义,评价其安全性。结果:中药组临床疗效明显优于西药组(73.3%VS 50.0%)(P<0.05);治疗后两组患儿发热、皮疹、口腔黏膜疱疹评分均明显下降,血清IL-6、TNF-α、IL-10含量均降低;中药组上述改善均较西药组明显(P<0.05);两组患儿均未见明显不良反应。结论:加味解毒清火合剂治疗小儿手足口病湿热蒸盛型疗效显著,可明显降低血清炎性因子水平,安全性高。Objective:To explore the clinical effect of Modified Jiedu Qinghuo Mixture on children with hand,foot and mouth disease with hot and humid steaming,and the effects on serum interleukin IL-6,IL-10,TNF-α.Methods:60 children with hand,foot and mouth disease were randomly divided into Chinese medicine group and Western medicine group with 30 cases in each group,the Chinese medicine group was treated by oral administration of Modified Jiedu Qinghuo Mixture,while the Western medicine group was treated by intravenous drip of Ribavirin Injection.To evaluate the clinical effect of continuous medication for 3~6 days.Before and after treatment,observe the improvement of main symptoms such as fever,rash and herpes of oral mucosa,the contents of IL-6,IL-10 and TNF-αin serum were measured by ELISA,comparing the significance of change,and evaluate d safety.Results:The clinical effect of Chinese medicine group was better than that of Western medicine group(73.3%vs 50.0%)(P<0.05).After treatment,the scores of fever,rash and herpes of oral mucosa in the two groups were decreased significantly,the serum levels of IL-6、IL-10 and TNF-αwere significantly decreased,the above improvements in the Chinese medicine group were more significant than those in the Western medicine group(P<0.05).No obvious adverse reactions were found in the two groups.Conclusion:The clinical effect of modified Jiedu Qinghuo Mixture is significant on children with hand,foot and mouth disease with hot and humid steaming,it can significantly reduce the level of serum inflammatory factors,the security of the method is good.
