肺保护性通气治疗创伤性急性呼吸窘迫综合征的效果探讨

摘　　要：目的探讨肺保护性通气治疗严重胸部创伤并发急性呼吸窘迫综合征的临床效果。方法纳入医院收治的80例严重胸部创伤并发急性呼吸窘迫综合征患者,选取时间为2018年5月—2019年5月,按照盲选方法将其分为两组。对照组40例采用传统正压通气治疗,研究组40例采用肺保护性通气治疗,对比两组的疗效和血气指标。结果研究组的总有效率大于对照组(P<0.05)。与治疗前相比,两组治疗后的动脉血氧分压(PaO2)、动脉二氧化碳分压(PaCO2)以及血氧饱和度(SpO2)指标均有所提高(P<0.05);且与对照组相比,研究组治疗后的血气指标提升更明显(P<0.05)。结论肺保护性通气治疗严重胸部创伤并发急性呼吸窘迫综合征疗效可靠。Objective To investigate the clinical effect of lung protective ventilation in the treatment of severe thoracic trauma complicated with acute respiratory distress syndrome.Methods 80 patients with severe thoracic trauma complicated with acute respiratory distress syndrome were enrolled in our hospital.They were selected from May 2018 to May 2019 and divided into two groups according to the blind selection method.40 cases in the control group were treated with traditional positive pressure ventilation,and 40 cases in the study group were treated with lung protective ventilation.The curative effect and indexes of blood gas of the two groups was compared.Results The total effective rate of the study group was greater than that of the control group(P<0.05).Compared with before treatment,the indexes of arterial partial pressure of oxygen(PaO2),arterial par t ial pressure of carbon dioxide(PaCO2)and blood oxygen saturation(SpO2)in both groups were improved after treatment(P<0.05),and the indexes of blood gas in the study group were increased more significantly after treatment than those in the control group(P<0.05).Conclusion Pulmonary protective ventilation is effective in the treatment of severe thoracic trauma complicated with acute respiratory distress syndrome.
