基于俞募配穴理论观察电针对功能性肠病双向调节作用

摘　　要：目的基于俞募配穴理论观察电针治疗两种不同状态(腹泻、便秘)的功能性肠病临床疗效。方法 118例功能性肠病的患者分为腹泻组60例和便秘组58例。两组均取双侧天枢、大肠俞,得气后连接电针治疗,连续治疗4周。观察两组不同时间自主排便次数、粪便性状评分变化,并比较两组临床疗效。结果腹泻组的总有效率为95.0%,便秘组为91.4%,两组总有效率比较,差异有统计学意义(P<0.05)。治疗第3周,两组患者的自主排便次数、粪便性状评分均有效改善,治疗后1周、治疗后3周、治疗后5周,两组患者的自主排便次数、粪便性状评分均明显改善,差异有统计学意义(P<0.05)。结论俞募配穴电针对功能性肠病具有双向调节作用;对功能性腹泻及功能性便秘的近、远期疗效均显著,但其对功能性腹泻的治疗作用较功能性便秘更为明显。Objective To observe the clinical efficacy of electroacupuncture in treating two different types(diarrhea and constipation)of functional bowel disease(FBD)based on the back-Shu and front-Mu acupoint grouping theory.Method A total of 118 FBD patients were divided into a diarrhea group of 60 cases and a constipation group of 58 cases.Bilateral Tianshu(ST25)and Dachangshu(BL25)were selected in both groups and connected to electroacupuncture after needling qi was achieved.The treatment lasted for 4 consecutive weeks.The changes in the spontaneous bowel movements and stool form score were observed at different time points in the two groups,and the clinical efficacy was compared between the two groups.Result The total effective rate was 95.0%in the diarrhea group versus 91.4%in the constipation group,and the difference was statistically significant in the total effective rate between the two groups(P<0.05).At treatment week 3,the spontaneous bowel movements and stool form score showed improvements in both groups;1 week,3 weeks and 5 weeks after treatment,the spontaneous bowel movements and stool form score still showed marked improvements,and the differences were statistically significant(P<0.05).Conclusion Electroacupuncture based on back-Shu and front-Mu acupoint grouping can produce bidirectional regulatory effect on FBD;it can produce significant short-term and long-term efficacy for both functional diarrhea and functional constipation,while the efficacy for functional diarrhea is more significant than that for functional constipation.
