从幽门螺杆菌感染视角新解“邪之所凑,其气必虚”

摘　　要：以幽门螺杆菌(Hp)感染为例,从Hp感染的因-机-证-治-防角度对《黄帝内经》中“邪之所凑,其气必虚”的理论进行新的阐述。Hp属中医“邪气”范畴,Hp感染病因病机有两个方面:一是脏腑功能失调,脾胃受损,正气亏虚则易感染Hp,此即“正虚”为因,“邪凑”为果。二是Hp感染日久导致脾胃受损,脏腑功能失常,造成正气亏虚,进而使病情发展,此即“邪凑”为因,“正虚”为果。“邪凑”与“正虚”不能单纯强调“正虚”为因,“邪凑”为果,需要全面理解其因果关系。防治Hp相关疾病应扶正与祛邪并举。Taking Helicobacter pylori(Hp)as an example,this paper reinterpreted the theory of"where the evil is,its Qi will be deficient"in Inner Canon of Yellow Emperor from the perspective of etiology-pathogenesis-syndrome-treatment-prevention of Hp.Hp belongs to the category of"pathogenic Qi"in traditional Chinese medicine(TCM).The etiology and pathogenesis of Hp infection have two aspects:one is the dysfunction of the zang-fu organs.The spleen and stomach are damaged,and deficiency of vital Qi are vulnerable to Hp infection.“Vital Qi deficiency”is the cause and“pathogens”is the result.The second is that long-term Hp infection causes damage to the spleen and stomach,dysfunction of viscera,resulting in vital Qi deficiency,as well as progress of disease.Here“pathogens”is the cause,and“vital Qi deficiency”is the result."Pathogens"and"vital Qi"cannot simply emphasize"vital Qi"as the cause,and"pathogens"as the result.It is necessary to fully understand the causality.The prevention and treatment of Hp-related diseases should both strengthen the vital Qi and eliminate the pathogens.
