摘 要:目的:观察电针干预腰多裂肌损伤模型大鼠肥大细胞(MC)变化及肝细胞生长因子(HGF)、肝细胞生长因子激活剂(HGFA)和肝细胞生长因子受体(c-Met)蛋白表达影响。方法:选择54只雄性SD大鼠随机分为空白组、模型组、电针组,每组18只。以多裂肌肌注0.5%布比卡因建立腰多裂肌损伤模型。各电针组造模后24 h开始电针双侧“委中”穴,3组在治疗后1 d、3 d、7 d同步取双侧多裂肌。采用甲苯胺蓝法观察各组多裂肌、多裂肌皮肤、“肾俞”穴肥大细胞情况;Elisa检测各组多裂肌HGFA、HGF和c-Met蛋白表达水平。结果:甲基胺蓝染色显示多裂肌周围组织切片中模型组MC及脱颗粒数较其他两组改变明显。Elisa结果显示电针组治疗后1 d腰多裂肌HGF表达量高于模型组(P<0.01)。电针组治疗后1 d HGFA表达量低于模型组(P<0.01),治疗后3 d表达量差异无统计学意义(P>0.05),7 d电针组高于模型组(P<0.01)。治疗后3 d电针组c-Met表达高于模型组(P<0.01)。结论:电针“委中”穴降低多裂肌周围肥大细胞数及脱颗粒数,有抑制多裂肌损伤可能。肥大细胞脱颗粒使多裂肌周围环境改变,旁分泌因子HGF被HGFA激活,与c-Met受体结合,促腰多裂肌损伤修复。Objective:To observe the effect of electro-acupuncture on mast cells(MC),hepatocyte growth factor(HGF),hepatocyte growth factor activator(HGFA)and hepatocyte growth factor receptor(c-Met)in rats with lumbar multifidus muscle injury.Methods:54 male SD rats were randomly divided into the blank group,the model group and the electro-acupuncture group,with 18 rats in each group.The model of lumbar multifidus muscle injury was established by injection of 0.5%Bupivacaine.In the electro-acupuncture group,the rats were given the intervention of electro-needling Weizhong(BL40)24h after modeling.Bilateral multifidus muscles were collected after one day,three days,and seven days of the treatment in the three groups.The toluidine blue staining method was used to observe the morphology of multifidus muscle.The expression levels of HGFA,HGF and c-Met protein were detected by Elisa method in each group.Results:Toluidine blue staining results showed that MC and the degranulation were more obvious in the model group than those in the other two groups.Elisa results showed that the expression of HGF was higher in the electro-acupuncture group than that in the model group after one day of the treatment(P<0.01).The expression of HGFA was lower in the electro-acupuncture group than that in the model group after one day of the treatment(P<0.01);of which there was no statistical difference after three days of the treatment between the two groups(P>0.05),and the electro-acupuncture group was higher than the model group after seven days of the treatment(P<0.01).The expression of c-Met was higher in the electro-acupuncture group than that in the model group after three days of the treatment(P<0.01).Conclusion:Electro-needling BL40 could decrease MC and the degranulation around multifidus muscle,which could possibly inhibit the injury of multifidus muscle.The degranulation of MC made the environment of multifidus muscle change,activated HGF and HGFA to combine with c-Met,so as to promote the repair the injury of lumbar multifidus muscle.
