摘 要:目的探讨缺血修饰白蛋白(IMA)、高敏肌钙蛋白I(hs-cTnI)和肌酸激酶同工酶MB(CK-MB)对过敏性紫癜(HSP)患儿早期心肌损伤的临床应用价值。方法将137例HSP患儿分为心肌损伤组48例和无心肌损伤组89例,同期健康对照组50例,分别采用清蛋白钴结合、微粒子化学发光免疫法和免疫抑制法测定血清中IMA、hs-cTnI和CK-MB,比较三组间hs-cTnI、IMA和CKMB表达水平和阳性率,分析hs-cTnI、IMA和CKMB对HSP心肌损伤的诊断价值。结果心肌损伤组Hs-cTnI、IMA和CKMB水平及阳性率高于对照组和无心肌损伤组(P<0.05)。IMA、hs-cTnI和CKMB联合检测,可提高对早期心肌损伤的阳性检出率和阴性预测值(P<0.05)。结论敏感性高IMA和特异性强的hs-cTnI联合检测,同时结合CK-MB,有利于早期诊断HSP患儿的缺血性心肌损伤。Objective To investigate the application value of IMA,hs-cTnI and CK-MB in the diagnosis of myocardial injury in children with Henoch-Schonlein purpura(HSP).Methods One hundred and thirty-seven children with HSP were divided into myocardial injury group(n=48)and non-myocardial injury group(n=89).At the same time,50 healthy children were taken as control group.IMA,hs-cTnI and CKMB were detected by albumin cobalt binding test and microparticle chemiluminescence immunoassay and immuno-suppression,respectively.The expression levels and positive rates of hs-cTnI,IMA and CK-MB were compared among the three groups.The diagnostic value of hs-cTnI,IMA and CKMB in HSP myocardial injury was also compared.Results Levels and positive rate of IMA,CK-MB and hs-cTnI in the myocardial injury group were significantly higher than those in the healthy control group and non-myocardial injury group.The sensitivity and negative predictive value were improved by the combined of CXCL16 and C-MYC(P<0.05).Conclusion Combined high sensitive IMA with high specific hs-cTnI as well as CK-MB are helpful for early diagnosis of myocardial injury in patients with Henoch-Schonlein purpura.
