麦粒灸配合针刺治疗子宫腺肌症诱发痛经的临床观察

摘　　要：目的:观察麦粒灸配合针刺治疗子宫腺肌症诱发痛经的疗效及对炎症因子水平的影响。方法:将2017年8月-2018年9月滨州医学院附属医院门诊就诊的80例子宫腺肌症患者通过随机数字表法分为对照组(40例)和观察组(40例)。对照组给予左炔诺孕酮宫内节育系统治疗。观察组给予常规针刺和麦粒灸(子宫、次髎)治疗,月经前1周开始治疗至月经来潮,每天1次。治疗3个月经周期。记录观察并对比两组治疗后痛经程度、经量、子宫体积、子宫内膜、炎症因子(白细胞介素-6、前列腺素E2、前列腺素F2α)水平的变化。结果:观察组总有效率97.5%(39/40);对照组总有效率82.5%(33/40)。治疗后两组患者痛经程度、经量、子宫体积、子宫内膜、炎症因子水平均较治疗前均改善(P<0.05)。结论:麦粒灸联合针刺治疗子宫腺肌症,不仅痛经、月经情况明显改善,而且子宫体积、内膜厚度、炎症因子水平均良性改善,值得临床适用并推广。Objective:To observe the curative effect of wheat grain moxibustion combined with acupuncture on dysmenorrhea induced by adenomyosis and its effect on the level of inflammatory factors.Method:80 patients with adenomyosis treated in outpatient clinic(from August 2017 to September 2018)were divided into the control group(40 cases)and the observation group(40 cases).The control group was given intrauterine Levonorgestrel system treatment.The observation group was treated with routine acupuncture and wheat grain moxibustion Zigong(子宫)point and Ciliao(次髎)point.The treatment began one week before menstruation until menstruation,once a day.Treatment proceeded three menstrual cycles.The changes in the levels of dysmenorrhea,volume of menstrual,volume of uterine,endometrium,and inflammatory factors(interleukin-6,prostaglandin E2,prostaglandin F2α)were recorded and compared between the two groups.Result:The total efficiency of the observation group and the control group was 97.5%(39/40)and 82.5%(33/40)respectively.After treatment,dysmenorrhea,volume of menstrual,volume of uterine,endometrium,and levels of inflammatory factors in the two groups are improved(P<0.05).Conclusion:Moxibustion of wheat grains combined with acupuncture are used for treating uterine adenomyosis,not only menstrual pain and menstrual conditions have been significantly relieved,but also the volume of the uterus,thickness of the inner membrane,inflammation factors level started to improve,which is worthy of clinical application and promotion.
