摘　　要：肺结核属中医“肺痨”范畴,应用抗结核药物杀灭结核分枝杆菌(中医称“痨虫”)是主要的治疗方法,但近年来结核分枝杆菌的耐药率不断增加,尤其是对异烟肼、利福平具有耐药性的耐多药结核病以及广泛耐药结核病的出现,使结核病防治形势变得更加严峻。由于其抗痨治疗时间长、不良反应多、治疗费用高昂等,且患者常出现咯血、肺部感染、毁损肺等,严重影响患者的生活质量。广泛耐药结核病、全耐药结核病面临着无药可用的困境,因此探索中西医结合攻克耐多药结核病治疗难题越来越为人们所重视。已有研究显示,中医药辅助治疗结核病可改善患者临床症状,缩短结核杆菌转阴时间,提高痰菌阴转率,促进病灶吸收和空洞闭合,提高治愈率,改善患者生活质量。中医药治疗肺结核,辨证用药相对灵活,治疗方法多种多样,但没有统一辨证分型标准。其次,中药的有效成分及中药作用的“靶点”研究不甚明确,中药治疗耐药肺结核的作用机制需要进一步深入研究。中医药在治疗耐多药肺结核的研究,多数属于小样本的研究,实验设计不甚规范,缺乏大样本、多中心的临床研究,使得现有的研究成果难以得到推广和应用,不能得到广泛的认可。总结了近10年来中医药及中医适宜技术对耐多药肺结核治疗成果和研究进展,以便我们发掘优势,摒弃不足,将来能够更好地推广和运用中医适宜技术。Tuberculosis belongs to the category of"tuberculosis"in traditional Chinese medicine.The main treatment method is to kill mycobacterium tuberculosis with anti-tuberculosis drugs(known as"tuberculosis"in traditional Chinese medicine).However,the drug resistance rate of mycobacterium tuberculosis has been increasing in recent years,especially the emergence of isoniazid,rifampicinresistant multidrug-resistant tuberculosis and extensively drug-resistant tuberculosis,which makes the situation of tuberculosis prevention and treatment more serious.Because the treatment of multidrug-resistant tuberculosis has a long treatment time,many side effects and high cost,patients often appear hemoptysis,pulmonary infection,lung damage and so on,which seriously affects the quality of life of patients.The patients who have extensive drug-resistant tuberculosis or fully drug-resistant tuberculosis are facing the plight of no drugs.Therefore,people pay more and more attention to explore the combination of traditional Chinese and Western medicine in order to overcome the difficult problem of multi-drug resistant tuberculosis treatment.Previous studies have shown that the adjuvant treatment of tuberculosis with traditional Chinese medicine can relieve the clinical symptoms of patients,shorten the time of tuberculosis negative transformation,improve sputum negative conversion rate,promote focus absorption and cavity closure,and enhance the cure rate and the quality of life of patients.In the treatment of tuberculosis with traditional Chinese medicine,the differentiation of symptoms and signs is relatively flexible,and the treatment methods are various,but there is no unified standard of differentiation of symptoms and signs.Secondly,the effective ingredients of traditional Chinese medicine and the target of its action are not clear.It needs further study for the mechanism of Chinese medicine in treating drug-resistant tuberculosis.The research of traditional Chinese medicine in the treatment of multidrug resistant pulmonary tuberculosis
