摘　　要：目的探讨临床输血患者中Diego血型不规则抗体产生的阳性率及分布特征。方法采用柱凝集法对本省68 918例临床申请输血的患者筛查不规则抗体;对409例初筛阳性患者采用试管低离子盐溶液-抗球蛋白试验(LISS-IAT)法进行不规则抗体种类及特异性的鉴定;对19例Diego血型不规则抗体阳性的患者采用试管法进行ABO、Rh以及Diego血型鉴定。结果 19例Diego血型不规则抗体中,女性患者13例(68.42%),男性患者6例(31.58%);所检出的抗体均为IgG-Di^a抗体,占所有不规则抗体阳性患者的4.65%,阳性率为0.03%。疾病分类:肝胆系统疾病6例(31.58%),妇产科疾病6例(31.58%),血液系统疾病5例(26.32%),肾脏系统疾病1例(5.26%),心血管系统疾病1例(5.26%);ABO血型中,A型8例(42.11%)、B型5例(26.32%)、O型4例(21.05%)、AB型2例(10.52%);Rh血型系统中,DCCee 7例(36.84%)、DCcEe 5例(26.32%)、DCcee 4例(21.05%)、DccEE 3例(15.79%)。结论对临床申请输血的患者(尤其是妇产科患者)进行不规则抗体的筛查以及抗体特异性的鉴定,可有效预防新生儿溶血病的发生以及减少输血不良反应。Objective To explore the prevalence and profile of antibody to Diego blood group antigen(Di^a) in clinical patients required for blood transfusion in our province. Methods The irregular antibodies of 68 918 patients who applied for blood transfusion in our province were screened by column agglutination technique. The types and specificities of irregular antibodies in 409 patients who were positive in initial screening were identified using LISS-IAT method. As to 19 patients who present antibody to Di^a, ABO, Rh and Diego blood groups of them were identified by tube method. Results Anti-Di^a was found in 19 patients required for blood transfusion, including 13 female(68.42%) and 6 male(31.58%). All anti-Di^a detected out were IgG-Di^a, with a frequency of 4.65%(19/409) among patients presented irregular antibodies and 0.03%(19/68 918) among all patients screened. Disease profile in anti-Di^a: hepatobiliary disease was found in 6 cases(31.58%), obstetrics and gynecology disease in 6(31.58%), blood disease in 5(26.32%), kidney disease in 1(5.26%), and cardiovascular disease in 1(5.26%). As to ABO blood group profile in anti-Di^a: type A, B, O and AB were found in 8(42.11%), 5(26.32%), 4(21.05%) and 2(10.52%) cases;Rh blood group profile: DCCee, DCcEe, DCcee and DccEE were found in 7(36.84%), 5(26.32%), 4(21.05%) and 3(15.79%) cases. Conclusion The screening and specificity identification of irregular antibodies in patients(especially patients from Obstetrics and Gynecology Department) applying for blood transfusion can effectively prevent the occurrence of hemolytic disease of newborn and reduce the adverse reactions of blood transfusion.
