摘 要:目的分析昆明市“三位一体”结核病防治模式运行效果,为政府制定结核病防控策略提供科学依据。方法收集昆明市全面实施“三位一体”的14个县(市)区前一年(2016年1-12月)和后一年(2018年1-12月)肺结核患者登记、总体到位、就诊延迟、病原学阳性诊断情况、治疗转归、免费抗结核药品固定剂量复合制剂使用等数据,采用SPSS 17.0软件进行分析,探讨两种模式的运行效果。结果“三位一体”转型前后,登记率分别为30.15/10万和35.23/10万,(χ^2=26.558,P<0.05)、总体到位率分别为94.54%和95.41%,(χ^2=21.384,P<0.05)、病原学阳性率分别为36.58%和44.37%,(χ^2=10.643,P<0.05)、免费抗结核药品FDC使用率分别为68.36%和85.31%,(χ^2=23.058,P<0.05)两者之间的差异均有统计学意义。“三位一体”转型前后成功治疗率分别为94.23%和89.39%,(χ^2=1.179,P>0.05)、转型前后患者发病到就诊的平均时间分别为(86.69±6.950)天和(76.20±8.953)天,(t=0.9,P>0.05),两者之间的差异均无统计学意义。结论昆明市“三位一体”结核病防治模式运行良好,符合新形势下结核病防治工作需要。Objective To analyze the effect of the“Trinity”tuberculosis prevention and control model in Kunming,and to provide a scientific basis for the government to formulate prevention and control strategies for tuberculosis.Methods The data of tuberculosis patients registration,overall arrival,delayed treatment,positive pathogen diagnosis,treatment outcome,use of free anti-tuberculosis drug(fixed-dose combination preparation),etc.of the 14 counties(cities)districts was collected where the“Trinity”was fully implemented in Kunming fromthe previous year(January to December,2016)and the following year(January to December,2018).It was then analyzed with SPSS 17.0 software and the effect of the two modes was discussed.Results Before and after the“Trinity”transformation,the registration rates were 30.15/100,000 and 35.23/100,000,(χ^2=6.558,P<0.05),and the overall arrival rate were 94.54%and 95.41%,(χ^2=21.384,P<0.05),and the pathogenic positive rates were 36.58%and 44.37%,(χ^2=10.643,P<0.05),the use of free anti-tuberculosis drugs(fixed-dose combination preparation)were 68.36%and 85.31%,(χ^2=23.058,P<0.05).The difference between the two is statistically significant.Before and after the“Trinity”transformation,the successful treatment rates were 94.23%and 89.39%,(χ^2=1.179,P>0.05),and the average time from onset to visit was(86.69±6.950)days and(76.20±8.953)days,(t=0.9,P>0.05),the difference between the two was not statistically significant.Conclusion The“Trinity”tuberculosis prevention and control model in Kunming is running well,which is in line with the needs of tuberculosis prevention and treatment under the new situation.
