手法复位小夹板固定联合石氏伤科三色敷药治疗Colles骨折的临床研究

摘　　要：目的:观察手法复位小夹板固定联合石氏伤科三色敷药治疗Colles骨折的临床疗效。方法:将80例Colles骨折患者随机分为两组,对照组40例患者进行手法复位小夹板外固定术治疗,治疗组40例进行手法复位小夹板固定配合石氏伤科三色敷药治疗,前2周每3 d调整小夹板松紧程度及更换三色敷药,2周后每周复诊,总固定时间为6周。比较两组患者腕关节疼痛(NRS)和肿胀的程度,以及患者第3个月腕关节功能情况。结果:2周时治疗组患者腕关节疼痛和肿胀的程度明显低于对照组,差异有统计学意义(P<0.05);治疗组患者骨折愈合时间短于对照组(P<0.05);3个月时治疗组Gartland-Werley腕关节功能评分优良率优于对照组,差异有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:手法复位小夹板外固定联合石氏伤科三色敷药治疗桡骨远端骨折临床疗效较好。Objective: To observe the clinical efficacy of manual reduction and small splint fixation combined with Shi’s tricolor medicinal paste in the treatment of Colles fracture. Methods: The 80 cases of patients with Colles fracture were selected, then were divided into two groups randomly, control group patients(40 cases)were treated with manual reduction and small splint external fixation, the observation group(40 cases) were given Shi’s tricolor medicinal paste on the basis of control group. The tightness of the splint was adjusted every 3 days and tricolor dressing was changed. The total fixed time was 6 weeks. The degree of anterior and posterior wrist pain(NRS) and swelling, as well as wrist function in the third month were compared between the two groups. Results: At 2 weeks, the degree of wrist pain and swelling in the observation group was significantly lower than that in the control group, and the difference was statistically significant(P <0.05). Fracture healing time in the observation group was shorter than that in the control group(P<0.05). At 3 months, the excellent and good rate of Gartland-Werley wrist function score in the observation group was better than that in the control group, and the difference was statistically significant(P<0.05). There was no difference in the incidence of adverse reactions between the two groups. Conclusion: Manipulative reduction and external fixation with small splint combined with Shi’s tricolor medicinal paste in the treatment of distal radius fracture is of superior clinical efficacy.
