回肠造口术后刺激性接触性皮炎合并切口感染的相关影响因素及防治策略

摘　　要：目的探讨回肠造口术后刺激性接触性皮炎合并切口感染的相关影响因素以指导制定针对性防治策略。方法选取2018年1月至2019年7月于我院进行回肠造口术的患者172例为研究对象,采用调查问卷收集患者的相关资料,包括性别、年龄、文化程度、体质量指数、婚姻状态、照顾者、疾病种类、基础疾病、造口方式、引流方式、术后时间、造口指导,通过单因素、多因素logistic回归分析回肠造口术后刺激性接触性皮炎合并切口感染的相关影响因素。结果172例回肠造口术后患者刺激性接触性皮炎合并切口感染25例。单因素分析提示,年龄≥60岁、体质量指数≥25 kg/m^2、照顾者为其他人员、疾病种类为肿瘤、有基础疾病、造口方式为袢式造口、引流方式为腹腔引流、术后时间≥15 d、未定期进行造口指导是回肠造口术后患者刺激性接触性皮炎合并切口感染的因素(P<0.05)。多因素logistic回归分析显示,体质量指数≥25 kg/m^2、照顾者为其他人员、有基础疾病、造口方式为袢式造口、引流方式为腹腔引流、术后时间≥15 d、未定期进行造口指导是回肠造口术后刺激性接触性皮炎合并切口感染的独立危险因素(P<0.05)。结论回肠造口术后刺激性接触性皮炎合并切口感染的发生受到体质量指数、照顾者、基础疾病、造口方式、引流方式、术后时间、造口指导多种因素影响,临床工作者应综合以上各因素制定合理的防治措施,降低刺激性接触性皮炎合并切口感染的发生率,提高患者术后生活质量。Objective To explore the related factors of irritant contact dermatitis combined with incision infection after ileostomy to guide the formulation of targeted prevention and treatment strategies.Methods We selected 172 cases of patients who underwent ileostomy in our hospital from January 2018 to July 2019 as the study objects.A questionnaire was used to collect relevant information of patients,including gender,age,education level,body mass index,marital status,caregiver,type of disease,underlying diseases,ostomy method,drainage methods,postoperative time,and stoma guidance.Univariate and multivariate logistic regression analysis was used to analyze the related influencing factors effecting irritant contact dermatitis combined with incision infection after ileostomy.Results There were 25 cases of combined incision infection in 172 cases of patients with irritant contact dermatitis after ileostomy.Univariate analysis showed that the age≥60 years old,body mass index≥25 kg/m^2,caregiver was other personnel,the type of disease was tumor,the underlying disease was present,the ostomy method was a loop stoma,the drainage method was abdominal drainage,postoperative time≥15 d,and failure to perform stoma guidance on a regular basis were the factors effecting the irritating contact dermatitis combined with incision infection in patients after ileostomy(P<0.05).Multivariate logistic regression analysis showed that the the body mass index≥25 kg/m^2,caregiver was other personnel,the underlying disease was present,the ostomy method was a loop stoma,the drainage method was abdominal drainage,postoperative time≥15 d,and failure to perform stoma guidance on a regular basis were the independent risk factors effecting the irritating contact dermatitis combined with incision infection in patients after ileostomy(P<0.05).Conclusion The occurrence of irritant contact dermatitis combined with incision infection after ileostomy was affected by various factors such as body mass index,caregiver,underlying diseases,ostomy method,d
