摘  要:目的旨在调查和分析非透析性慢性肾病(CKD)患者室性心律失常的发生率及其预测因素,为预防和治疗提供相关依据。方法连续入选于西安交通大学第一附属医院诊治的CKD患者,并行24 h动态心电图、心脏超声及相关实验室检查,对室性心律失常进行分析。结果84例CKD患者(男性53例)中34例(40%)发现室性心律失常,其发生与高龄、肾小球滤过率(GFR)降低、左心室质量及左心功能受损密切相关。经logistic多因素分析显示,高龄、GFR水平和左室射血分数下降是此类患者室性心律失常发生的预测因素。结论非透析CKD患者有较高发生室性心律失常的风险,在临床治疗过程中需要密切监测并进行早期防治,以最大程度地降低其发生率。Objective To investigate and analyze the incidence and predictive factors of ventricular arrhythmias in patients with nondialysis chronic kidney disease(CKD),and to provide relevant evidence for prevention and treatment.Methods Continuously selected CKD patients diagnosed and treated in our hospital,and examined 24-hour dynamic electrocardiogram,cardiac ultrasound and related laboratory examinations to analyze ventricular arrhythmia.Results Of the 84 CKD patients(53 males),34(40%)found ventricular arrhythmias,which occurred closely with the elderly,decreased glomerular filtration rate(GFR),left ventricular mass and left ventricular function impairment Related.Logistic multivariate analysis showed that advanced age,GFR level and decreased left ventricular ejection fraction are the predictors of ventricular arrhythmia in these patients.Conclusion Non-dialysis CKD patients have a higher risk of developing ventricular arrhythmias.In the course of clinical treatment,close monitoring and early prevention and treatment are required to minimize their incidence.
