摘  要:目的探讨红花黄色素注射液对AECOPD合并肺动脉高压患者的治疗作用,以及对血流动力学和氧化应激反应的影响。方法选择2018年4月~2020年1月我院诊治的100例AECOPD合并肺动脉高压患者,随机分为红花黄色素+常规治疗组(干预组)和常规治疗组(对照组),各50例。分别测定治疗前及治疗7天后患者动脉血气指标(pH、PO2和PCO2)、FEV1、FEV1/FVC、血浆ET-1、TXB2、MDA和SOD水平,并使用彩色多普勒超声心动图(CDE)测定射血分数(EF)、平均肺动脉压(mPAP)、吸气相上腔静脉最大前向血流速度(VINS)、呼气相肺活量最大前向血流速度(VEXP)以及VINS/VEXP比值。结果治疗后,两组动脉血气指标(pH、PO2和PCO2)、肺功能指标(FEV1和FEV1/FVC)、血清ET-1、TXB2、MDA和SOD水平、EF、mPAP、VINS、VEXP和VINS/VEXP均较治疗前改善(P>0.05),且与对照组相比较,干预组动脉血气指标(pH、PO2和PCO2)、肺功能指标、血浆ET-1、TXB2、MDA和SOD水平、EF%、mPAP、VINS、VEXP和VINS/VEXP改善(P<0.05)。同时该实验过程中干预组患者未出现与红花黄色素注射液相关的严重药物不良反应。结论红花黄色素注射液对AECOPD相关的肺动脉高压异常血流动力学具有显著的改善作用,同时红花黄色素注射液可有效抑制AECOPD诱导的过度氧化应激反应,对于治疗AECOPD相关的肺动脉高压和慢性肺源性心脏病安全有效。Objective To explore the therapeutic effect of safflower yellow injection on patients with AECOPD complicated with pulmonary hypertension,as well as its effect on hemodynamics and oxidative stress.Methods A total of 100 patients with AECOPD and pulmonary hypertension diagnosed and treated in our hospital from April 2018 to January 2020 were randomly divided into safflower yellow+conventional treatment group(intervention group)and conventional treatment group(control group),each with 50 cases.The arterial blood gas indexes(pH,PO2 and PCO2),FEV1%,FEV1/FVC,plasma ET-1,TXB2,MDA and SOD levels were measured before treatment and 7 d after treatment,and color Doppler echocardiography was used(CDE)measured ejection fraction(EF),mean pulmonary artery pressure(mPAP),maximum forward blood velocity of the superior vena cava during inspiration(VINS),maximum forward blood velocity of expiratory vital capacity(VEXP),and VINS/VEXP ratio.Results After treatment,the two groups of arterial blood gas indicators(pH,PO2 and PCO2),lung function indicators(FEV1 and FEV1/FVC),serum ET-1,TXB2,MDA and SOD levels,EF,mPAP,VINS,VEXP and VINS/VEXP were improved compared with before treatment(P>0.05),and compared with the control group,the intervention group arterial blood gas indicators(pH,PO2 and PCO2),lung function indicators,plasma ET-1,TXB2,MDA and SOD levels,EF,mPAP,VINS,VEXP and VINS/VEXP improved significantly(P<0.05).At the same time,patients in the intervention group did not have serious adverse drug reactions related to safflower yellow injection during the experiment.Conclusion Safflower yellow injection could significantly improve the abnormal hemodynamics of AECOPD-related pulmonary hypertension.At the same time,safflower yellow injection could effectively inhibit the excessive oxidative stress induced by AECOPD,which was useful for the treatment of AECOPD-related pulmonary hypertension and chronic cor pulmonale was safe and effective.
