摘　　要：目的探讨全肌肉分离单孔胸腔镜治疗自发性气胸的临床效果。方法选取本院2019年1月至2019年7月收治的单纯自发性气胸患者62例,按照随机数字表法分为观察组与对照组,各31例。观察组采用全肌肉分离单孔胸腔镜,对照组采用传统肌肉切开单孔胸腔镜。比较两组治疗疗效、手术指标、术后指标、术后并发症及术后24 h视觉模拟评分法(VAS)评分。结果两组治愈率比较差异无统计学意义。观察组术中出血量少于对照组(P<0.05);两组手术时间比较差异无统计学意义。两组术后总引流量、留管时间和住院时间比较差异无统计学意义。两组术后并发症发生率比较差异无统计学意义。观察组术后24 h VAS评分低于对照组(P<0.05)。结论全肌肉分离单孔胸腔镜治疗自发性气胸患者可降低术中出血量,减轻疼痛,值得临床推广。Objective To explore the clinical effect of single-port thoracoscopy with whole muscle separation for spontaneous pneumothorax.Methods 62 patients with simple spontaneous pneumothorax admitted to our hospital from January 2019 to July 2019 were divided into observation group and control group according to random number table method,with 31 cases in each group.The observation group was treated with single-hole thoracoscopy with whole muscle separation,while the control group was treated with traditional single-hole thoracoscopy with muscle incision.The therapeutic effect,operative index,postoperative index,complications and VAS score of 24 hours after operation were compared between the two groups.Results There was no significant difference in the cure rate between the two groups.The amount of bleeding in observation group was significantly less than control group(P<0.05),and there was no significant difference in operation time between the two groups.There was no significant difference in total drainage,tube retention time and hospitalization time between the two groups.There was no significant difference in postoperative complications between the two groups.The VAS score in observation group was significantly lower than control group 24 hours after operation(P<0.05).Conclusion Single-port thoracoscopy with whole muscle separation can reduce the amount of bleeding and pain in patients with spontaneous pneumothorax,which is worthy of clinical promotation.
