超敏CRP、D二聚体、LP(α)联合诊断冠心病的应用价值分析

摘　　要：目的探讨超敏C反应蛋白(hs-CRP)、D二聚体(DD)、脂蛋白(α)[LP(α)]联合诊断冠心病的应用价值。方法选取2018年10月至2019年3月本院收治的84例高度疑似冠心病作为观察组,另选取同期于本院体检心脏结构和功能正常者84名作为对照组。所有受检者均进行hs-CRP、DD、LP(α)水平检测,观察两组血清中3种因子水平变化情况,并统计观察组各指标对诊断冠心病的阳性检出率。结果观察组hs-CRP、LP(α)以及DD水平明显高于对照组,差异有统计学意义(P<0.05)。经金标准冠脉造影检查后确诊,84例疑似冠心病患者中,82例确诊为冠心病。单独采用hs-CRP对诊断冠心病的阳性检出率为68.29%(56/82),单独采用LP(α)对诊断冠心病的阳性检出率为79.27%(65/82),单独采用DD对诊断冠心病的阳性检出率为86.59%(71/82),联合检测对诊断冠心病的阳性检出率96.34%(79/82),hs-CRP、LP(α)及DD联合检测对诊断冠心病的阳性检出率均高于单独检测,差异有统计学意义(P<0.05)。结论冠心病患者血清中hs-CRP、DD、LP(α)水平呈高表达,三者联合检测冠心病的阳性检出率较高,对冠心病的早期预防、治疗及预后具有较高的诊断价值。Objective To explore the value of combined diagnosis of coronary heart disease with hypersensitive C-reactive protein(hs-CRP),DDimer(DD),lipoprotein(α)[Lp(α)].Methods From October 2018 to March 2019,84 patients with highly suspected coronary heart disease in our hospital were selected as the observation group.84 medical examinees who had normal cardiac structure and function at the same time in our hospital were selected as the control group.The levels of hs-CRP,DD and LP(α)were detected in all the subjects.The changes of serum levels of three factors in the two groups were observed,and the positive detection rate of various indicators for the diagnosis of coronary heart disease in the observation group was counted.Results The levels of hs CRP,LP(α)and DD in the observation group were higher than those in the control group,the difference was statistically significant(P<0.05).After being diagnosed by gold standard coronary angiography,all the 84 patients with suspected coronary heart disease,82 were diagnosed with coronary heart disease.The positive detection rate of coronary heart disease with hs-CRP alone was 68.29%(56/82),the positive detection rate of coronary heart disease with LP(α)alone was 79.27%(65/82),and the positive detection rate for coronary heart disease with DD alone was 86.59%(71/82),the positive detection rate of coronary heart disease with the combined detection was 96.34%(79/82),the combined detection of hs-CRP,LP(α)and DD of positive detection rate of diagnosis of coronary heart disease was higher than that of single detection,and the difference was statistically significant(P<0.05).Conclusion The levels of hs CRP,DD and LP(α)in the serum of patients with coronary heart disease are highly expressed,and the positive rate of combined detection of the three is high,which has high diagnostic value for the early prevention,treatment and prognosis of coronary heart disease.
