摘　　要：目的探究来氟米特联合泼尼松治疗狼疮性肾炎的效果及对24 h尿蛋白定量、血肌酐水平的影响。方法选取2017年至2019年本院收治的78例狼疮性肾炎患者作为研究对象,以随机抽签的方法将患者分为对照组和观察组,每组39例。对照组采取环磷酰胺联合泼尼松治疗,研究组采取来氟米特联合泼尼松治疗。比较两组患者的24 h尿蛋白定量、血肌酐水平治疗前后的变化以及治疗期间的不良反应发生情况。结果治疗前,两组患者24 h尿蛋白定量水平以及血肌酐水平比较差异无统计学意义。治疗后,研究组24 h尿蛋白定量水平以及血肌酐水平均显著低于对照组(P<0.05)。治疗期间,研究组恶心呕吐发生率、转氨酶增高发生率、脱发发生率以及白细胞下降发生率均显著低于对照组,差异有统计学意义(P<0.05)。结论将来氟米特联合泼尼松治疗方案应用于狼疮性肾炎患者的治疗中,治疗效果良好,能够进一步降低患者24 h尿蛋白定量以及血肌酐水平,提高治疗安全性,值得临床推广。Objective To investigate the effect of leflunomide combined with prednisone in the treatment of lupus nephritis and its effect on 24-hour urine protein quantitation and serum creatinine level.Methods 78 patients with lupus nephritis admitted to our hospital from 2017 to 2019 were selected as the study subjects,and the patients were divided into the control group and the study group by random drawing,with 39 cases in each group.The control group was treated with cyclophosphamide and prednisone,and the study group was treated with leflunomide and prednisone.The 24-hour urine protein quantification,changes in blood creatinine level before and after treatment and the incidence of adverse reactions during treatment were compared between the two groups.Results There was no statistically significant difference in quantitative urine protein levels and serum creatinine levels between the two groups of patients 24 hours before treatment.After treatment,the quantitative urine protein level and serum creatinine level in the study group at 24 h were significantly lower than those in the control group(P<0.05).During the treatment period,the incidence of nausea and vomiting,the increased incidence of transaminase,the incidence of alopecia,and the incidence of white blood cell decline in the study group were significantly lower than in the control group(P<0.05).Conclusion Leflunomide combined with prednisone treatment scheme used in the treatment of patients with lupus nephritis can obtain good therapeutic effects,which can further reduce the 24-hour urine protein and blood creatinine levels of patients and improve the treatment safety.It is worth promoting.
