摘　　要：目的:分析游轮聚集性新型冠状病毒肺炎(COVID-19)的临床表现和辅助检查特点。方法:回顾性分析2020年1月20日至3月9日于本院就诊的“世纪梦号”游轮上COVID-19患者9例的临床资料,收集患者的一般资料、临床表现、实验室检查和胸部CT检查结果并进行分析。结果:(1)共收治游轮上感染的4个家庭9例COVID-19患者,1个家庭共3例患者来源于武汉,其他均来源于广州;传播途径以呼吸道飞沫为主;人群普遍易感,尤其有基础疾病者。所有患者疾病潜伏期长短不一,但都在离开游轮14 d内。(2)患者临床症状普遍较轻,均有咳嗽;咽干或咽痛5例,发热、乏力较少见。(3)2例患者血气分析示氧分压下降,肝功能轻度异常5例,所有患者肾功能均基本正常;咽拭子新型冠状病毒(2019-nCoV)核酸检测均为阳性,存在支气管内粘液栓的1例患者核酸检测持续未转阴。(4)大部分患者呈病毒性肺炎典型CT改变,武汉患者和有基础疾病者CT改变较明显。结论:COVID-19聚集性发病风险较高,潜伏期长短不一,有公共场所传染源接触史者需早隔离、早诊断、早治疗。Objective:To analyze the clinical manifestations and auxiliary examination characteristics of patients with novel coronavirus pneumonia in cruise ship cluster infection.Methods:Retrospective analysis was performed on the clinical data of 9 covid-19 patients on the"Century dream"cruise ship who was admitted to our hospital from January 20 to March 9,2020.General information,clinical manifestations,laboratory and chest CT findings of the patients were collected and analyzed.Results:(1)A total of 9 COVID-19 patients from 4 families infected on the cruise ship were admitted and treated,including 3 patients from Wuhan and the others from Guangzhou.Respiratory droplets were the main route of transmission.People were generally susceptible,especially those with basic diseases.All patients had varying incubation periods,but all were within 14 days off debarkation.(2)The clinical symptoms of the patients were generally mild,and all of them had cough.Dry or sore throat in 5 cases and fever or fatigue was rare.(3)Blood gas analysis of 2 patients showed lowered oxygen partial pressure,mild abnormal liver function in 5 patients,and basically normal renal function in all patients.Novel Coronavirus(2019-nCoV)nucleic acid tests of pharyngeal swabs were all positive,and 1 patient with bronchial mucus plug did not turn negative continuously.(4)Most of the patients presented typical CT changes of viral pneumonia,and the patients from Wuhan and those with underlying diseases were more serious.Conclusion:The risk of COVID-19 clustering outbreak is high and the incubation period varies.People with a history of exposure to sources of infection in public places should be isolated,diagnosed and treated early.
