摘　　要：目的:探讨高龄、高危患者一站式杂交手术治疗A型主动脉夹层的疗效。方法:回顾性分析2018年9月至今本院收治的8例患者,其中男性6例,女性2例,均行一站式杂交手术,比较分析其主动脉阻断时间、体外循环时间、术后呼吸机使用时间,监护室时间及术后并发症情况。结果:8例手术均成功,主动脉阻断时间(62±9.8)min,体外循环时间(135±16.4)min。术后呼吸机使用时间(43±35.9)h,监护室时间(96±42.6)h,术后均康复出院,复查未见明显异常。结论:杂交手术避免了高风险的深低温停循环,创伤较小,手术时间较常规手术时间短,在高龄、高危A型夹层治疗效果明确,但中远期效果仍需继续观察。Objective:To investigate the efficacy of one-stop hybrid surgery for type A aortic dissection in elderly and high-risk patients.Methods:A retrospective analysis was performed of eight patients referred to our hospital so far from September 2018,including six males and two females.All the patients underwent one-stop hybrid surgery.The aortic cross clamp time,cardiopulmonary bypass(CPB)duration,and postoperative hours of ventilator use,length of ICU stay,and complications were compared among the patients.Results:The surgery was successful in all eight cases.The aortic cross clamp time was(62±9.8)min,the CPB duration was(135±16.4)min,postoperative hours of ventilator use was(43±35.9)h,and length of ICU stay was(96±42.6)h.All patients recovered and were discharged home after operation.Follow up examinations were unremarkable.Conclusion:Hybrid surgery averts the high-risk of deep hypothermic circulatory arrest,and is with less trauma and shorter operation time compared to conventional surgery,thereby offering obvious efficacy for type A aortic dissection in elderly and high-risk patients albeit pending further validation for the mid-and long-term efficacy.
