摘 要:在儿童重症救治中,气道管理是一个具有挑战性的难题,气管插管可以挽救生命。儿童气道变异较大,氧储备较低,合适的导管深度尤为关键。导管放置过浅会导致声带损伤、意外拔管、低氧血症甚至死亡,放置过深(即支气管内)可能导致肺不张或创伤性肺损伤。文章阐述了儿童气道的特点及导管深度的定义,基于国内外儿童气管插管深度的研究现状,归纳不同导管深度定位法的特点,总结当前确定导管深度的新方法,分析新方法的应用现状及存在的缺点,并对超声定位导管深度的方法进行展望。Airway management is an enormous challenge in the critical care of children.Intubation is one way of saving life in the cases of emergency.In children,the airway has significant variations while the oxygen reserve of kids is low.Thus,the appropriate catheter depth is particularly critical.Shallow catheter placement probably leads to vocal cord injury,accidental extubation,hypoxemia and even death.Over-depth placement(such as endobronchial)may result in atelectasis or traumatic lung injury.This review discusses the characteristics of children's airway and the definition of catheter depth.This paper summarize the characteristics of different localization methods of children's tracheal catheter positioning based on universal literature review.And it also demonstrate the new methods of catheter positioning,advantages and disadvantages of these new methods,and future direction for ultrasound guided catheter placement in children.
