溃疡性结肠炎S型回肠储袋肛管吻合术一例报道

摘　　要：回肠储袋肛管吻合术(IPAA)多采用J型回肠储袋,在系膜长度足够的情况下进行无张力吻合。我们报道1例溃疡性结肠炎患者术中采用离断回结肠血管以延长系膜长度,拟构建J型储袋吻合时,发现系膜长度略短导致无法完成无张力肛管吻合,遂改为构建S型储袋,增加了2 cm系膜长度,顺利完成IPAA手术。J-pouch is mostly used in ileal pouch-anal anastomosis(IPAA),and tension-free anastomosis is performed when the length of mesentery is sufficient.A case with ulcerative colitis undergoing S-pouch during IPAA procedure was reported.When the J-pouch was to be constructed for anastomosis,it was found that the length of the mesentery was slightly shorter and the tension-free anal anastomosis could not be completed.Therefore,the ileocolic vessels were cut to prolong the mesentery for more than 2 cm,then the S-pouch was constructed instead.The IPAA was successfully completed.
