曲美他嗪联合养心通脉方治疗冠状动脉慢血流现象的疗效及对心室复极化的影响

摘　　要：目的探讨曲美他嗪联合养心通脉方治疗冠状动脉慢血流(CSF)现象的疗效及对心室复极化的影响。方法选择2017年1月—2018年12月内蒙古医科大学附属医院治疗的CSF患者65例,随机分为2组,对照组32例在常规治疗基础上口服曲美他嗪,观察组33例在对照组治疗基础上给予养心通脉方治疗,2组疗程均为3个月。记录2组干预前后冠脉血管的校正TIMI帧数(CTFC)、心室复极化指标。统计2组临床疗效及治疗后随访6个月时的心血管不良事件(MACE)发生率。结果与治疗前比较,治疗后2组左前降支(LAD)、左回旋支(LCX)、右冠状动脉(RCA)的CTFC均明显降低(P均<0.05),且治疗后观察组上述冠脉血管CTFC均低于对照组(P均<0.05)。与治疗前比较,治疗后2组T波峰末间期(Tp-e)、校正后的QT间期(QTc)、T波峰末间期与校正后的QT间期比值(Tp-e/QTc)均明显降低(P均<0.05),且观察组治疗后Tp-e、QTc、Tp-e/QTc均明显低于对照组(P均<0.05)。观察组总有效率为90.9%(30/33),对照组为71.9%(23/32),观察组明显高于对照组(P<0.05)。2组治疗后均随访6个月,观察组MACE总发生率为9.1%(3/33),对照组为25.0%(8/32),观察组明显低于对照组(P<0.05)。结论曲美他嗪联合养心通脉方可有效缓解CSF患者症状及慢血流状况,改善心室复极化。Objective It is to investigate the curative effect of trimetazidine combined with Yangxin Tongmai Decoction on coronary slow flow(CSF)and its influence on ventricular repolarization.Methods 65 CSF patients treated in the Affiliated Hospital of Inner Mongolia Medical University from January 2017 to December 2018 were selected and randomly divided into 2 groups.32 cases in the control group were treated with trimetazidine orally on the basis of conventional treatment,and 33 cases in the observation group were treated with Yangxin Tongmai Decoction on the basis of treatment in the control group,the course of treatment for both groups was 3 months.The corrected TIMI frame number(CTFC)of the coronary arteries and ventricular repolarization indexes before and after the intervention were recorded in the two groups.The clinical efficacy of the two groups and the incidence of adverse cardiovascular events(MACE)during the 6-month follow-up period were calculated.Results Compared with before treatment,the CTFC of the left anterior descending branch(LAD),left circumflex branch(LCX),and right coronary artery(RCA)in the two groups were significantly reduced after treatment(P<0.05),and the CTFC of the above-mentioned coronary vessels in the observation group after treatment were lower than those in the control group(P<0.05).Compared with before treatment,T wave end interval(Tp-e),corrected QT interval(QTc),Tp-e/QTc were significantly reduced after treatment(P<0.05),and Tp-e,QTc,and Tp-e/QTc in the observation group were significantly lower than those in the control group(P<0.05).The total effective rate of the observation group was 90.9%(30/33),and that of the control group was 71.9%(23/32),the rate of the observation group was significantly higher than that of the control group(P<0.05).Both groups were followed up for 6 months after treatment.The total incidence of MACE in the observation group was 9.1%(3/33),and that in the control group was 25.0%(8/32),the observation group was significantly lower than the control group(P<0.
