摘  要:目的探讨在B超引导下MST行经外周静脉置入中心静脉导管(PICC)钝性分离法和改良扩皮法的应用效果。方法选取2019年1月至6月在我院PICC中心置管的患者216例,随机分为对照组110例和观察组106例,对照组使用钝性分离法进行扩皮,观察组采用将钝性分离法和传统刀片扩皮法结合的改良方法进行扩皮。观察并比较两组的一次性送鞘成功率,皮下出血发生率,操作用时以及两组患者渗血、渗液的发生率。结果观察组一次性送鞘成功率明显高于对照组,皮下出血发生率低于对照组,操作用时短于对照组(P<0.05),两组渗血、渗液发生率差异无统计学意义(P>0.05)。结论改良扩皮法的一次性送鞘成功率高,可减轻渗血、渗液以及皮下出血等并发症,缩短操作用时。Objective To investigate the application effect of blunt separation and modified skin expansion method of peripherally inserted central venous catheter( PICC) in MST under the guidance of B-ultrasound. Methods From January to June2019,216 patients who were placed in PICC center of our hospital were selected that randomly divided into control group( CG,n =110) and observation group( OG,n = 106).CG used blunt separation method to expand skin,while OG used modified method combining blunt separation and traditional blade expansion method to expand skin.The success rate of one-time delivery of sheath,the incidence of subcutaneous hemorrhage,the operation time and the incidence of blood and exudate in two groups were observed and compared.Results Compared to CG,the one-time delivery success rate of OG was significantly higher,the incidence of subcutaneous hemorrhage was lower,and the operation time was shorter( P<0. 05).There was no significant difference between two groups in the incidence of bleeding and exudation( P>0. 05).Conclusion The modified skin expansion method has a high success rate for one-time delivery of the sheath,which could reduce complications such as blood oozing,fluid leakage and subcutaneous hemorrhage,and shorten the operation time.
