摘  要:原发性脑损伤是由外力直接或间接暴力产生的颅脑损伤,严重的原发性脑损伤病理生理过程十分复杂,往往发生严重的脑移位、脑水肿,导致颅内压增高,产生脑缺血缺氧从而导致预后不良。继发性脑损伤是在原发性颅脑损伤的基础上,由于脑组织缺血缺氧而导致的内环境紊乱、炎症因子产生、细胞坏死凋亡、轴突再生抑制等参与到继发性脑损害的发生机制中。创伤性颅脑损伤(TBI)后相关的脑组织血管损伤以及相关的血流动力学改变在继发性脑损害中非常基础,且相当重要,本文对此进行综述,以引起TBI救治中的重视。Primary brain injury is caused by direct or indirect external force,the pathophysiological process of severe primary brain injury is very complex,which often leads to serious brain displacement and brain edema,which leads to the increase of intracranial pressure,the occurrence of cerebral ischemia and hypoxia and the poor prognosis.Secondary brain injury is the internal environment disorder caused by cerebral ischemia and hypoxia on the basis of primary brain injury,the production of inflammatory factors,cell necrosis and apoptosis,axon regeneration inhibition are involved in the mechanism of secondary brain damage.After TBI,the related cerebral vascular injury and hemodynamic changes are very basic and important in the secondary brain injury,in this paper,we summarize this in order to pay attention to TBI treatment.
