摘 要:目的探讨新型冠状病毒肺炎(COVID-19)患者胸部影像学变化及患者临床检测指标与影像学评分的相关性。方法回顾性分析18例确诊COVID-19患者的临床资料。其中,普通型13例,重型4例,危重型1例。采用常规技术检测血常规、凝血功能、血生化和血气分析,行胸部CT检测。分析COVID-19患者的胸部影像学评分与临床检测指标的相关性。结果COVID-19患者影像学主要为两肺多发斑片状磨玻璃阴影和实变影,沿支气管血管和胸膜下分布,进展期肺部影像学表现为磨玻璃阴影、实变和结节等多种情况。18例患者影像学评分为3(1.75,7.25)分,与淋巴细胞计数呈负相关(r=-0.677,P<0.05),与乳酸脱氢酶、D-D和C反应蛋白呈正相关(r=0.673、0.654和0.758,P<0.05)。结论血乳酸脱氢酶、D-D、C反应蛋白、淋巴细胞计数改变可作为评估COVID-19患者病情的参考指标。Objective To analyze the correlation between the clinical indicators and imaging scores in the patients with coronavirus disease 2019(COVID-19).Methods The clinical data of 18 patients with COVID-19 were retrospectively analyzed,who were common type in 13 cases,severe type in 4 cases,and was critical type in one case.The blood routine examination,blood coagulation function,blood biochemistry,blood gas analysis and chest CT examination were performed using conventional techniques.The correlation between chest imaging scores and clinical detection indexes in COVID-19 patients was analyzed.Results The imaging of COVID-19 patients mainly showed multiple patchy ground glass shadows and consolidation in both lungs,which were distributed along the bronchial blood vessels and the subpleura.The lung imaging in progression stage manifested by ground glass shadows,consolidation,and nodules.The imaging score of 18 patients was 3(1.75,7.25)points,which was negatively correlated with lymphocyte count(r=-0.677,P<0.05),and positively correlated with lactate dehydrogenase,D-D and C-reactive protein(r=0.673,0.654 and 0.758,P<0.05).Conclusion The levels of blood lactate dehydrogenase,D-D,C-reactive protein,and lymphocyte counts can be used as reference indexes for evaluating the illness status of the patients with COVID-19.
