摘 要:目的观察分次球囊扩张技术治疗伴严重骨质疏松Kümmell病的临床疗效。方法84例伴严重骨质疏松的Ⅰ型或Ⅱ型Kümmell病患者随机分为两组,40例行分次球囊扩张技术治疗(分次球囊扩张组),44例行传统球囊扩张技术治疗(传统球囊扩张组)。术后第1天和3个月行VAS疼痛评分和Oswestry功能障碍指数(ODI)评分,根据影像学检查结果比较两组骨水泥渗漏情况。结果两组术后第1天和3个月VAS疼痛评分和ODI评分均较术前降低(P<0.05),两组术后第1天与3个月VAS疼痛评分、ODI评分比较有统计学差异(P<0.05),但两组间VAS疼痛评分和ODI评分比较无统计学差异(P>0.05)。分次球囊扩张组骨水泥渗漏发生率低于传统球囊扩张组(7.5%vs.25.0%)(P<0.05)。结论对于伴严重骨质疏松的Ⅰ型或Ⅱ型Kümmell病,分次球囊扩张技术和传统球囊扩张技术均有较好的临床疗效。与传统球囊扩张技术相比,分次球囊扩张技术可降低骨水泥渗漏的发生率。Objective To observe the clinical efficacy of multiple balloon expansion technique in the treatment of Kümmell’s disease combined with severe osteoporosis.Methods A total of 84 patients with typeⅠorⅡKümmell’s disease combined with severe osteoporosis was treated with multiple balloon expansion technique(group A,40 cases)or conventional balloon expansion technique(group B,44 cases).The patients were followed up on the 1st day and in the 3rd month after operation,during which the oswestry disability index score(ODI)and VAS pain score were evaluated.The bone cement leakage of the two groups was diagnosed by imaging examination.Results The ODI score and VAS pain score were all lower on the 1stday and in the 3rd month after operation than those before in the two groups(P<0.05),which were not significantly different between the two groups(P>0.05).The incidence of the bone cement leakage was lower in group A than that in group B(7.5%vs.25.0%)(P<0.05).Conclusion Both groups have good clinical effect and can effectively relieve pain,multiple balloon expansion technique is better than conventional balloon expansion technique in reducing the incidence of bone cement leakage in the treatment of typeⅠorⅡKümmell’s disease combined with severe osteoporosis.
